this completed form in to the school counseling office. Make sure to select the “flex” flvs option.

_____ You must have enough classes to take at DA to fill your 8 period schedule in addition to the class(es)
you take on FLVS until you graduate. Student’s must have minimum 3 academic courses.

_____ There is no guarantee that you will be able to take the planned course next year due to various
circumstances such as class sizes or schedule conflicts. Therefore, you may have to take a class which
is not your preference such as an additional academic class.
