AFFIDAVIT OF ATTACHMENT OF A SIDECAR

I, SERAFIN L. ALMARVEZ, JR., of legal age, Filipino, married and a

resident of Sitio 6, Brgy. Palasan, Santa Cruz, Laguna after having been
sworn in accordance with law hereby depose and state that –

1. That I bought from Desmark Corporation a Honda Motorcycle

with Sales Invoice No. SCL-00000000970 dated 27 November
2013, more particularly described as follows –

Series: CB125 Kick
Motor No.: CGX125SHD-SBM
Chassis No.: KYY20003305
Plate No.: New

2. That I attached a sidecar on the above motorcycle for service

purposes.

3. That sidecar was bought from Mr. Lito Malacoco of Brgy. San
Pablo Norte, Santa Cruz, Laguna.

4. That I executed this affidavit to attest to the truth of the foregoing

facts and for whatever purposes this may legally serve.
