(of witness LISA KIM)

I, LISA A. KIM, after being duly sworn to in accordance with law,

hereby depose and state that I am executing this Judicial Affidavit to
substantiate the instant complaint.

The person examining me is Atty. MINHO LEE with office address at

No. 01 Dalan na Pagayaya, Carig Sur, Tuguegarao City and this examination
was conducted at his residence at 02 Carig Norte, Tuguegarao City. The
questions asked are in English, a language which I speak and understand and
I answered likewise in. And fully conscious that I did so under oath and that I
am aware that I may face criminal liability if proven that I gave false
testimony or perjured the statements I made.

Atty. Lee: The testimony of the witness is being offered:

: To prove matters material and relevant to the instant case.

: The foregoing Judicial Affidavit is likewise being offered and

submitted in lieu of her testimony in direct examination as may
be allowed by this Honorable Court and in compliance with
A.M. No. 11-6-10-SC.

1. Q: State your name and other personal circumstances for the record.
A: I am Lisa A. Kim, 28 years old, single, and with residence at 123
Seoul Residences, South St. Makati City.

2. Q: What is the nature of your business?

A: I am an event coordinator and the owner of the Bella Creative
Events.

3. Q: How do you know the parties in this case?

A: They contacted me and asked to be their wedding coordinator.
4. Q: Did you sign a contract for this service?
A: Yes, sir.

5. Q: Showing to you this “Contract of Services” marked as Annex “A”,

are you referring to this?
A: Yes, sir.

6. Q: What are your responsibilities written in the contract as a wedding

coordinator of the parties?
A: I was asked to forefront the entire wedding preparation- from the
choosing of venue until the flower arrangements. I was tasked to
prepare everything for the parties.

7. Q: How much was your service fee?

A: It was ₱90,000, sir.

8. Q: How was your payment agreement?

A: We agreed that the payment of the services would be paid in two
separate occasions.

9. Q: When was the first payment? How much?

A: The first payment was last April 30, 2015. Ms. Fiona Quinto paid me
₱45,000.

10. Q: Showing to you this receipt marked as Annex “A.1”, are you
referring to this?
A: Yes, sir.

11. Q: After the payment, what were the services you rendered to the
parties?
A: I presented options for the venue of the wedding. One is the St.
Therese of Child Jesus Parish in Cebu. The other option is Balesin
Resort.

12. Q: What did they choose?

A: Ms. Quinto chose The Balesin Resort, sir.

13. Q: What else did you do after that?

A: No more, sir.

14. Q: Do you want to add anything else?

A: None sir.

1. I was the one who conducted the examination of Lisa Kim at my

aforementioned residence at 02 Carig Norte, Tuguegarao City;

2. I have faithfully recorded or caused to be recorded the

questions I asked and the corresponding answer that the witness gave;

3. I nor any other person then present or assisting her coached the
witness regarding her answers;
