Purpose
This form is an application for validation/verification bodies (V/VBs) that seek accreditation under ISO 14065:2013 - Greenhouse gases – Requirements for
greenhouse gas validation and verification bodies for use in accreditation and other forms of recognition from the American National Standards Institute (ANSI)
Accreditation Program for Greenhouse Gas Validation/Verification Bodies.
Upon receipt of all necessary items, ANSI will review the application and notify the V/VB of the outcome within 30 days. ANSI may: a) determine that the V/VB is
eligible to proceed with the application process and approve the application; b) request additional information or clarification; or c) determine that the V/VB is not
eligible to apply. If additional information or clarification is needed, the V/VB will have 90 days to submit it along with an updated version of this form.

Declaration Statement
Applicant agrees to meet the following conditions:
1. Applicant shall provide ANSI with all information requested for the process of assessing the validation/verification program(s);
2. Applicant is familiar with the requirements for assessment of validation/verification programs (e.g., ANSI requirements, ANSI/ISO 14065, ANSI/ISO 14064-3, applicable IAF Guidance on the
application of ANSI/ISO 14065, and other applicable standards and guidance, i.e. ISO/IEC Guide 65, ISO/IEC 17020, ISO/IEC 17025, etc.);
3. Applicant agrees to fulfill ANSI requirements for accreditation and the following obligations:
a. The Applicant shall commit to fulfill continually the ANSI requirements for accreditation for the areas where accreditation is sought;
b. When requested, the Applicant shall afford such accommodation and cooperation as is necessary to enable the accreditation body to verify fulfillment of requirements for
accreditation. This applies to all premises where the conformity assessment services take place;
c. The Applicant shall provide access to information, documents and records as necessary for the initial, surveillance and witness assessments as defined by the ANSI program;
d. The Applicant shall provide access to those documents that provide insight into the level of independence and impartiality of the Applicant from any related bodies, where
applicable;
e. The Applicant shall arrange the witnessing of Applicant services when requested by the accreditation body;
f. The Applicant shall pay fees as shall be determined by the accreditation body;
4. Applicant understands and acknowledges ANSI accreditation program requirements may change. The applicant shall commit to making any required program changes in order to obtain or
retain accreditation after any site visit(s);
5. Applicant understands and acknowledges that failure to provide full and accurate information may result in this application being delayed, rendered invalid or denied.
I accept the conditions aforementioned and attach said completed application for accreditation review by the American National Standards Institute.
