GUIDANCE ON COMPLETING THIS FORM AND THE APPLICATION PROCESS

Purpose
This form is an application for validation/verification bodies (V/VBs) that seek accreditation under ISO 14065:2013 - Greenhouse gases – Requirements for
greenhouse gas validation and verification bodies for use in accreditation and other forms of recognition from the American National Standards Institute (ANSI)
Accreditation Program for Greenhouse Gas Validation/Verification Bodies.

Before completing the Application

Before applying, a V/VB must first be approved as a preliminary applicant (see form GHG-FR-703). V/VBs are encouraged to review the applicable standards and
requirements before completing this form. Program information and links to ISO 14065, ISO 14064-3 - Greenhouse gases – Part 3: Specification with guidance for
the validation and verification of greenhouse gas assertions, and IAF MD 6 - IAF Mandatory Document for the Application of ISO 14065 are located on the GHG
Program section of the ANSI Accreditation Services web site. V/VBs are also advised to obtain and read a copy of GHG-FR-705 and the additional requirements of
the GHG program(s) for which they intend to provide services.

Completing the Application

Fully complete all sections in clear type and include all information required. Do not modify this form other than filling in response cells. This form must be
submitted electronically along with a copy of the quality and verification manuals (with relevant procedures and policies), the $5,000 application fee, GHG-FR-705
with all referenced documentation, and supporting documentation as indicated in this form.

Process for reviewing the Application

Receipt of payment for the application as well as the execution of ANSI program agreement shall be required before the application is processed. ANSI will confirm
receipt of the complete application. If acknowledgement of receipt is not received within five working days, please contact ANSI staff.

Upon receipt of all necessary items, ANSI will review the application and notify the V/VB of the outcome within 30 days. ANSI may: a) determine that the V/VB is
eligible to proceed with the application process and approve the application; b) request additional information or clarification; or c) determine that the V/VB is not
eligible to apply. If additional information or clarification is needed, the V/VB will have 90 days to submit it along with an updated version of this form.

Approval of the Application

Once the application is approved, ANSI will propose an assessment team and an Evaluation Task Group (ETG) comprised of at least two members of our
Greenhouse Gas Validation/Verification Accreditation Committee (GVAC). Once the assessment team and ETG are assigned, the next step in the initial
assessment process, the document review, will commence.
