Erythrocytes at a Glance:
Pieces of human erythrocytes have a diameter of about 6-8 μm and thickness of 2 μm, smaller than
the other cells contained in the human body. Normal erythrocytes have a volume of about 9 FL (9
femtoliter) About a third of the volume filled by hemoglobin, a total of 270 million hemoglobin
molecules, where each molecule carrying four heme groups.
Adults have 2-3 × 1013 red blood cells every time (women have a 4-5 million erythrocytes per
microlitre of blood and men to have 5-6 million. While the people who live in the highlands who
have low oxygen levels will tend to have red blood cells more numerous). Erythrocytes in the blood
contained high amounts compared with other blood particles, such as white blood cells which only
has about 4000-11000 white blood cells and platelets that have only 150000-400000 in every
microlitre in human blood.
In humans, the hemoglobin in red blood cells have a role to deliver more than 98% oxygen
throughout the body, while the remaining dissolved in the blood plasma.
Erythrocytes in the human body stores about 2.5 grams of iron, representing about 65% iron content
in the human body.

target cells are flat cells with large diameter and visible, these cells can be seen in thalassemia,
obstructive disease, sickle cell disease, etc.
"Sickle cell" are erythrocytes that looks like a crescent moon or clurit.sel can be found in the "sickle
cell disease", or other hemoglobinopathy.

crenate erythrocyte cell is visible to the wall "jagged" because of the bumps cytoplasm and
dispersed uniformly dull surface cells, most commonly due to technical problems in the manufacture
of smears.

Poikilositosis
ialah keadaan dimana populasi eritrosit tampil dengan bentuk yang bervariasi.biasanya polkilositosis
biasanya bersamaan dengan anisositosis.meningkatnya poikilositosis sering menunjukkan adanya
kelainan eritropoiesis yang disebabkan oleh defek sumsum tulang atau kelainan destruksi eritrosit.
dalam situasi normal,suatu poikilosit merupakan penuaan eritrosit yang sejalan dengan
kekuatannya.sebagian kecil dari membrannya terkelupas.dalam situasi yang abnormal,poikilositosis
menjadi sedemikian nyata sehingga eritrosit berbentuk tetesan airmata ("teardrops").gambaran ini
menjadi ciri dari eritropoiesis ekstrameduler.
sferofit
sel ini adalah eritrosit yang tidak lagi berbentuk bikonkaf tetapi bentuknya bulat (sferik) dengan
diameter kurang dari 6 um.dengan kata lain,volume sel berkurang sedang dindingnya menjadi
lebihtebal.oleh sebab itu pada sediaan apus sel ini tampak tidak memiliki akromia sentral dan warna
lebih atau sangat gelap dari warna normalnya,disebut mikrosperofit hiper kromik.
kelainan bentuk sel ini terjadi karena terganggunya fungsi membran sel.walaupun gangguan ini
dapat disebabkan oleh banyak hal tetapi sperositosis sering dijumpai pada kelainan bawaan
sperositosis herediter dimana terjadi kemacetan dalam mekanisme "sodium pump"nya gangguan
lain adalah "immuneinduced hemolysis"

sel target adalah sel yang pipih dengan diameter besar dan kelihatan,sel ini dapat terlihat pada
talasemia,penyakit obstruktif,penyakit sel sabit,dll

stomatitis (Inflammation of the corners of the mouth so that it appears as patches of pale whitish),
koilonychia (spoon nails , nails become brittle, vertical stripes and become concave)

a. Anemia defisiensi besi : disfagia (nyeri menelan karena kerusakan epitel hipofaring), atrofi
papil lidah (permukaan lidah menjadi licin can mengkilap karena papil lidah menghilang) ,
stomatitis angularis ( keradangan pada sudut mulut sehingga tampak sebagai bercak
berwarna pucat keputihan), koilonychia ( kuku sendok, kuku menjadi rapuh, bergaris-garis
vertikal dan menjadi cekung)
b. Anemia defisiensi asam folat : lidah merah (buffy tongue)
c. Anemia hemolitik : ikterus dan hepatosplenomegali
d. Anemia aplastik : perdarahan kulit atau mukosa dan tanda-tanda infeksi
Anemia resulting from empty body iron reserves (depleted iron stores) so that the supply of
iron to eritropoesis reduced, which in turn decreases the formation of hemoglobin
Etiology:
a. Iron loss as a result of chronic bleeding, which can be derived from: gastrointestinal
(stomach cancer, colon cancer, hemorrhoids, hookworm infection), female genital tract
(menorrhagia), bladder (hematuria), airway (hemoptoe)
b. Nutritional factors: a lack of the total amount of iron in the diet, or the quality of the iron
(bioavailibitas) iron is not good (lots of fiber foods, low in vitamin C, and low in meat)
Pathogenesis:
Chronic bleeding causes loss of iron so iron reserves decline. If the reserve is empty dining
condition called iron depleted state. If iron deficiency continues the supply of iron to
eritropoesis reduced so called iron deficiency anemia. At this very moment there is a
shortage of iron in the epithelium and in some enzymes that can meniimbulkan yoke
symptoms, oral and pharyngeal epithelium and various other symptoms.
Etiology:
Pathogenesis:
Smeua estimated above changes caused by the influence of proinflammatory cytokines, IL-1
and TNF-alpha on eritropoesis. Impaired release of iron into the plasma cause a reduction in
the supply of iron to eritropesis which resulted in the formation of hemoglobin disorders
causing anemia hypochromic mikrositer.
Pathophysiology:
Failure of iron incorporation into heme compounds and mitokhondria resulting precipitate
iron on mitokhondria so if painted with metal paint will appear as spots surrounding the
nucleus called ring sideroblasts. This led to the failure of the formation of hemoglobin which
is accompanied eritropoesis ineffective and cause anemia hypochromic mikrositer.
b. Megaloblastic Anemia
Anemia characterized by the presence of cells in the bone marrow Megaloblast. Tues
Megaloblast is erythrocyte precursor cells to form large cell maturation along the gap
cytoplasm and nucleus, cytoplasm maturasinya where normal but large nuclei with a loose
arrangement of chromosomes. Megaloblastic Anemia caused by impaired formation of the
DNA in the nucleus eritroblast, mainly due to deficiency of vitamin B12 and folic acid.
Pathogenesis: caused by a deficiency of vitamin B12 and folic acid, in which vitamin B12 and
folic acid.
Tues Megaloblast This function is not normal, destroyed stage in the bone marrow
(hemolysis intramedular) resulting in eritropoesis infective and erythrocyte life span is
shorter, resulting in anemia.
Anemia perniciosa
A megaloblastic anemia due to severe atrophy of the gastric mucosa, including parietal cells
so there is no secretion of intrinsic factor. The reason for the autoimmune process: the
emergence of antibodies against parietal cells.
c. Hemolytic Anemia
Hemolytic anemia arising from the formation of autoantibodies against its own erythrocytes,
causing hemolysis of erythrocytes.
Hemolysis caused by certain pathological processes that lead to full capillary fibrin that
erythrocytes are forced through a narrow hole. The result is damage to the fragmentation of
erythrocyte membranes.
Caused by:
anemia occurs because of the failure of bone marrow function. Bone marrow failure not
only disrupt the erythroid, myeloid and tetaoi megakriosit systems also giving rise to
pancytopenia. Were included in this group are:
I. Primary
II. Secondary
1. Anemia mieloptisik
Anemia due to foreign cell infiltration (especially malignant tumors) in the bone marrow
Decreased eritropoesisi
c. Eritropoesis inadequate response against each stimulus anemia, possibly due to impaired
production of eritropetin, or receptor interference
d. Gastrointestinal Bleeding
h. Hypersplenism
i. Abnormalities of lipid metabolism that wall erythrocytes become loose resulting in the
emergence of thin macrocyte until Spurr cell. These cells are easily difagosit by
macrophages.
4. hypothyroidism
a. Filters Test: This test is done at an early stage in any case of anemia. With this test it can
be ascertained the presence of anemia and anemia morphology.
These checks include:
i. hemoglobin levels
ii. Erythrocyte indices (MCV, MCH and MCHC). With the development of electronic counting
in the field of hematology then proceeds Hb, WBC (white blood) and PLT (platelets) and
erythrocyte indices can be seen at once. This new circuitry checks the well known RDW (red
cell distribution width) that indicate the level of red blood cells anisitosis
iii. Peripheral blood smear
b. Routine Inspection: This inspection is also done in all cases of anemia, to determine
abnormalities in the system of leukocytes and platelets. Examination should be done is:
i. Erythrocyte sedimentation rate
ii. differential count
iii. WCC
c. Bone marrow examination: This examination must be done in most cases to get a
definitive diagnosis of anemia although there are some cases that the diagnosis does not
require bone marrow examination.
d. The examination of a specific indication: This new inspection done if we had had a hunch
early diagnosis so that its function is to confirm the alleged diagnosis. Examination include:
e. Nonhematologik Laboratory tests: tests that need to be done include:
i. renal physiology
ii. endocrine physiology
iii. uric acid
iv. liver function
v. bacteria culture
vi. Etc.
Various types of anemia can be caused by systemic diseases such as chronic renal failure,
chronic liver disease, and hypothyroidism. There are also cases of anemia caused by the
basic disease accompanied by hyperuricemia, such as multiple myeloma. In the case of
sepsis accompanied by anemia, such as aplastic anemia required blood culture.
a. Tes Penyaring : tes ini dikerjakan pada tahap awal pada setiap kasus anemia. Dengan
pemeriksaan ini maka dapat dipastikan adanya anemia dan bentuk morfologi anemia
tersebut.
Pemeriksaan ini meliputi :
i. Kadar hemoglobin
ii. Indeks eritrosit (MCV, MCH dan MCHC). Dengan perkembangan electronic counting
di bidang hematologi maka hasil Hb, WBC (darah putih) dan PLT (trombosit) serta
indeks eritrosit dapat diketahui sekaligus. Dnegan pemeriksaan yang baru ini maka
juga diketahui RDW (red cell distribution width) yang menunjukkan tingkat anisitosis
sel darah merah
iii. Apusan darah tepi
b. Pemeriksaan rutin : pemeriksaan ini juga dikerjakan pada semua kasus anemia, untuk
mengetahui kelainan pada sistem leukosit dan trombosit. Pemeriksaan yang harus
dikerjakan adalah :
i. Laju endap darah
ii. Hitung diferensial
iii. Hitung leukosit
c. Pemeriksaan sumsum tulang : pemeriksaan ini harus dikerjakan pada sebagian besar kasus
anemia untuk mendapatkan diagnosis definitif meskipun ada beberapa kasus yang
diagnosisnya tidak memerlukan pemeriksaan sumsum tulang.
d. Pemeriksaan atas indikasi khusus : pemeriksaan ini baru dikerjakan jika kita telah
mempunyai dugaan diagnosis awal sehingga fungsinya adalah untuk mengkonfirmasi dugaan
diagnosis tersebut. Pemeriksaan tersebut antara lain :
e. Pemeriksaan laboratorium nonhematologik : pemeriksaan-pemeriksaan yang perlu
dikerjakan antara lain :
i. Faal ginjal
ii. Faal endokrin
iii. Asam urat
iv. Faal hati
v. Biakan kuman
vi. Dan lain-lain
Berbagai jenis anemia dapat disebabkan oleh penyakit sistemik seperti gagal ginjal kronik,
penyakit hati kronik, dan hipotiroidisme. Ada juga kasus anemia yang disebabkan oleh
penyakit dasar yang disertai hiperurisemia, seperti mieloma multipel. Pada kasus anemia
yang disertai sepsis, seperti pada anemia aplastik diperlukan kultur darah.
a. Anamnesis
As history in general, history in the case of anemia should be directed to explore:
- History of present illness
- History of previous illness
- Nutritional History
- The history of the environment, exposure to chemicals, and physical as well as a
history of drug use
- Family history
b. Physical examination
The physical examination should be conducted in a systematic and thorough. Special
attention is given to the following:
- Skin color: pale, plethora, cyanosis, jaundice, skin like a straw yellow palms
- Purpura: petechie and echymosis
- Nails: koilonychia (spoon nails)
- Eyes: jaundice, pale conjunctiva, peruabahan fundus
- Mouth: ulceration, hypertrophic gums, bleeding gums, tongue papil atrophy,
glossitis and angular stomatitis
- Lymphadenopathy
- Hepatomegaly
- Splenomegaly
- Bone pain or pain in the sternum
- Hemarthrosis or joint ankylosis
- Swelling of the testicles
- Swelling of the parotid
- Neurological abnormalities sisrem

a. Anamnesis
Seperti anamnesis pada umumnya, anamnesis pada kasus anemia harus ditujukan
untuk mengeksplorasi :
- Riwayat penyakit sekarang
- Riwayat penyakit terdahulu
- Riwayat gizi
- Anamnesis mengenai lingkungan, pemaparan bahan kimia, dan fisik serta riwayat
pemakaian obat
- Riwayat keluarga
b. Pemeriksaan Fisik
Pemeriksaan fisik harus dilakukan secara sistematik dan menyeluruh. Perhatian
khusus diberikan pada berikut :
- Warna kulit : pucat, plethora, sianosis, ikterus, kulit telapak tangan kuning seperti jerami
- Purpura : petechie dan echymosis
- Kuku : koilonychia (kuku sendok)
- Mata : ikterus, konjungtiva pucat, peruabahan fundus
- Mulut : ulserasi, hipertrofi gusi, perdarahan gusi, atrofi papil lidah, glossitis dan
stomatitis angularis
- Limfadenopati
- Hepatomegali
- Splenomegali
- Nyeri tulang atau nyeri sternum
- Hemarthrosis atau ankilosis sendi
- Pembengkakan testis
- Pembengkakan parotis
- Kelainan sisrem saraf
Patogenesis :
b. Anemia Megaloblastik
Anemia yang khas ditandai oleh adanya sel megaloblast dalam sumsum tulang. Sel
megaloblast adalah sel prekursor eritrosit dengan bentuk sel yang besar disertai
adanya kesenjangan pematangan sitoplasma dan inti, di mana sitoplasma
maturasinya normal tetapi inti besar dengan susunan kromosom yang longgar.
Anemia megaloblastik disebabkan oleh gangguan pembentukan DNA pada inti
eritroblast, terutama akibat defisiensi vitamin B12 dan asam folat.
Patogenesis : disebabkan oleh terjadinya defisiensi vitamin B12 dan asam folat, di
mana vitamin B12 dan asam folat.
Sel megaloblast ini fungsinya tidak normal, dihancurkan semasih dalam sumsum
tulang (hemolisis intramedular) sehingga terjadi eritropoesis infektif dan masa hidup
eritrosit lebih pendek, yang berujung pada terjadinya anemia.
Anemia perniciosa
Suatu anemia megaloblastik karena atrofi berat mukosa gaster termasuk sel parietal
sehingga tidak ada sekresi faktor intrinsik. Penyebabnya karena proses autoimun :
timbulnya antibodi terhadap sel parietal.
c. Anemia Hemolitik
anemia terjadi karena kegagalan fungsi sumsum tulang. Gagal sumsum tulang bukan
hanya mengganggu sistem eritroid, tetaoi juga sistem mieloid dan megakriosit
sehingga menimbulkan pansitopenia. Yang termasuk dalam kelompok ini ialah :
I. Primer
1. Anemia aplastik dan pure red cell aplasoa
Anemia yang disertai oleh pansitopenia pada darah tepi yang
disebabkan oleh kelainan primer pada sumsum tulang dalam bentuk
aplasia atau hipoplasia tanpa adanya infiltrasi, supresi atau pendesakan
sumsum tulang. Karena sumsum tulang sebagian besar bersifat
hipoplastik, bukan aplastik total, maka anemia ini disebut juga sebagai
anemia hipoplastik
2. Anemia diseritropoetik kongenital
Anemia herediter refrakter yang ditandai oleh eritropoesis inefektif, sel
eritroid (eritroblas) berinti banyak dan hemokromatosis
3. Anemia pada sindroma mielodisplastik
II. Sekunder
1. Anemia mieloptisik
Anemia yang timbul karena infiltrasi sel asing (terutama tumor ganas)
dalam sumsum tulang
2. Supresi sumsum tulang karena keganasan hematologik lain : leukimia
akut, limfoma maligna dan mieloma multipel

Iron deficiency anemia: is a type of anemia caused by lack of iron for the formation of hemoglobin
(Hb). Each red blood cells that the average age of 120 days, containing less hemoglobin than normal
due to a lack of iron as a constituent. This type of anemia (iron anemiadefisiensi) is the cause of
developing anemia.
Anemia resulting from empty body iron reserves (depleted iron stores) so that the supply of

Etiology:

Pathogenesis:

shortage of iron in the epithelium and in some enzymes that can meniimbulkan yoke
symptoms, oral and pharyngeal epithelium and various other symptoms.
ii. Anemia of chronic disease
Etiology:
Pathogenesis:

b. Megaloblastic Anemia

Megaloblast is erythrocyte precursor cells to form large cell maturation along the gap
cytoplasm and nucleus, cytoplasm maturasinya where normal but large nuclei with a loose
arrangement of chromosomes. Megaloblastic Anemia caused by impaired formation of the
DNA in the nucleus eritroblast, mainly due to deficiency of vitamin B12 and folic acid.

c. Hemolytic Anemia
Hemolytic anemia arising from the formation of autoantibodies against its own erythrocytes,

b. Faktor nutrisi : akibat kurangnya jumlah besi total dalam makanan, atau kualitas besi
(bioavailibitas) besi yang tidak baik (makanan banyak serat, rendah vitamin C, dan rendah
daging)

dan TNF-alfa terhadap eritropoesis. Gangguan pelepasan besi ke plasma menyebabkan
berkurangnya penyediaan besi untuk eritropesis yang berakibat pada gangguan
pembentukan hemoglobin sehingga terjadi anemia hipokromik mikrositer.
Patofisiologi :
Terjadi kegagalan inkorporasi besi ke dalam senyawa hem dan mitokhondria yang
mengakibatkan besi mengendap pada mitokhondria sehingga jika dicat dengan cat besi akan
kelihatan sebagai bintik-bintik yang mengelilingi inti yang disebut sebagai sideroblas cincin.
Hal ini menyebabkan kegagalan pembentukan hemoglobin yang disertai eritropoesis
inefektif dan menimbulkan anemia hipokromik mikrositer.
b. Anemia Megaloblastik
Anemia yang khas ditandai oleh adanya sel megaloblast dalam sumsum tulang. Sel
megaloblast adalah sel prekursor eritrosit dengan bentuk sel yang besar disertai adanya
kesenjangan pematangan sitoplasma dan inti, di mana sitoplasma maturasinya normal tetapi
inti besar dengan susunan kromosom yang longgar. Anemia megaloblastik disebabkan oleh
gangguan pembentukan DNA pada inti eritroblast, terutama akibat defisiensi vitamin B12
dan asam folat.
Patogenesis : disebabkan oleh terjadinya defisiensi vitamin B12 dan asam folat, di mana
vitamin B12 dan asam folat.
Sel megaloblast ini fungsinya tidak normal, dihancurkan semasih dalam sumsum tulang
(hemolisis intramedular) sehingga terjadi eritropoesis infektif dan masa hidup eritrosit lebih
pendek, yang berujung pada terjadinya anemia.
Anemia perniciosa
Suatu anemia megaloblastik karena atrofi berat mukosa gaster termasuk sel parietal
sehingga tidak ada sekresi faktor intrinsik. Penyebabnya karena proses autoimun : timbulnya
antibodi terhadap sel parietal.
c. Anemia Hemolitik
Anemia yang disebabkan oleh proses hemolisis. Hemolisis adalah pemecahan eritrosit dalam
pembuluh darah sebelum waktunya.
Anemia hemolitik yang timbul karena terbentuknya autoantibodi terhadap eritrosit sendiri
sehingga menimbulkan hemolisis eritrosit.
Anemia hemolitik ekstrakorpuskuler nonimun yang disebabkan oleh faktor-faktor luar bukan
oleh proses imunologik, dimana eritrosit mengalami hemolisis prematur akibat : stres
mekanik, akibat infeksi/toksin atau bahan kimia.
Hemolisis terjadi akibat proses patologik tertentu yang menyebabkan kapiler penuh fibrin
sehingga eritrosit dipaksa melewati lubang yang sempit. Akibatnya terjadi kerusakan
membran sampai fragmentasi eritrosit.
Disebabkan oleh :
I. Primer
Anemia yang disertai oleh pansitopenia pada darah tepi yang disebabkan oleh kelainan
primer pada sumsum tulang dalam bentuk aplasia atau hipoplasia tanpa adanya infiltrasi,
supresi atau pendesakan sumsum tulang. Karena sumsum tulang sebagian besar bersifat
hipoplastik, bukan aplastik total, maka anemia ini disebut juga sebagai anemia hipoplastik
Anemia herediter refrakter yang ditandai oleh eritropoesis inefektif, sel eritroid (eritroblas)
berinti banyak dan hemokromatosis
II. Sekunder

Anemia yang timbul karena infiltrasi sel asing (terutama tumor ganas) dalam sumsum tulang

Klasifikasi thalassemia
Patofisiologi : pada dasarnya thalassemia beta timbul karena presipitasi ini membentuk
inclusion bodies yang menyebabkan lisis eritrosit intrameduler dan berkurangnya masa
hidup sel eritrosit dalm sirkulasi.
a. Thalassemia major: bentuk homosigot dari thalassemia beta yang disertai anemia berat
dengan segala konsekuensinya. Gambaran kliniknya dapat dibagi menjadi 2 golongan
yaitu :
1. Yang mendapat transfusi yang baik sebagai akibat pemberian hipertransfusi maka
produksi HbF dan hiperplasia eritroid menurun sehingga anak tumbuh normal
sampai dekade ke 4-5. Setelah itu timbul gejala “iron overload” dan penderita
meninggal karena diabetes melitus atau sirosis hati.
2. Yang tidak mendapat transfusi yang baik maka timbul anemia yang khas, yaitu
Cooley’s anemia.
a. Gejala mulai pada saat bayi berumur 3-6 bulan, pucat, anemis, kurus,
hepatosplenomegali, dan ikterus ringan
b. Gangguan pada tulang
c. Rontgen tulang tengkorak : hair on end appearance
d. Gangguan pertumbuhan (kerdil)
e. Gejala iron overload : pigmentasi kulit, diabetes melitus, sirosis hati, atau
gonadal failure.
