NUM : Assalammualaikum Wr.Wb, Good morning everyone, thank you for your
attention and your time, according to the contract that we have been
agreed, today 17 May 2019 at 10.00 a.m, we will do nursing round.
Before we start it, I will introduce my team first, I am Nur Hidayanti as
NUM, Ria and Mila as primary nurse, Tiffani and Putri Mei as associate
nurse. This nursing round consists of case presentation, continued patient
validation, and back to ners station for evaluation and recommendation.
Our patient is An.N 8 month, BPJS class 3 with medical diagnose West
Syndrome + Severely Wasted + Microcephaly + ASD,with an unresolved
nursing problem is deficiency of nutrition. The goal of this round is to
discuss the patient's problem and then plan the right intervention for her.
For the next, please Ria as primary nurse (PN) 1 to explain the case or
patient problem.
PN1 : Thank you for the time given to me, and thanks to all the round teams that
have been present. Previously let me introduce my self, my name is Ria as
PN in Room Bona II, okay I will report the condition of patients in this
round, I start from the identity of patient ...........

This is a report of patient’s problems and the interventions we have done.

NUM : Well… thank you Ns. Ria for your presentation about the problems. Is
there any question or clarification? If no, then we just do the validation
to the patient to match the data with the patient problems. Lets go to the
patient. Ns. Ria, please prepare all the necessary, medical equipment and
medical report.

(the PN ask for help AN to prepare the equipments)

PN1 : Ners shindy, please prepare tension and medical equipments, medical
report and all data about An.N?
AN1 : Okay Ners, I’ll prepare it.
After the implementation of the nursing round at Ners Station, the round team visited
the patient's room for data validation

NUM : Good morning Mam, How’s the condition of An.N? Is she already getting
better?
An.N’s family : Good morning. not yet nurse, my daughter still cringing and crying
NUM : In accordance with the contract that we have agreed before, today there is a
nursing round. This nursing round is useful in order to solve the problem
of An.N condition so that her condition can improve faster. For the next
time please Ners Mila to do the assessment.
PN 2 : Good morning mom, sir. How’s the condition of An.N? is there any
grumble about the condition?
Family : Anak saya masih sering rewel dan berat badannya tidak naik-naik mbak.
AN 2 : Ners.. may I do the physical examination?
PN2 : Yes ners.. of course. Please do that correctly.
(AN do the physical examination)
(AN reports the data to the PN)

NUM : The examination has been completed by Ns. Putri Mei, After this we will
discuss it again for planning the action for X how to make him recover
quickly. Okay, let’s go to the ners station.

All team go to ners station for discussing about evaluation and recommendation
NUM : Okay we have validated to the patient, and we already get the data about
her, but now please come forward for Ns. Mila to explain the X’s
conditions from the validation.
PN 2 : Okay.. I will explain about An.N condition, from the assessment and
physical examination.. we found the patient is deficiency of nutrition.
Body mass index of patient is underweight. (Weight An.N is 4,2 Kg)
NUM : Ok thank you Ns. Mila, now how about the doctor or counselor maybe you
can explain about the patient’s conditions and add the recommendation or
suggestion for the intervention?
(doctor and counselor explain the patients conditions and give any suggestion)-----
DISCUSSION

NUM : Okay.. team for the recommendation please write it in integration notes so
there is documentation and we have list what should we do after this to
imptove X’s conditions. Okay.. if its clear, I will read back the results of
the intervention and recommendations earlier to clarify
(read the conclusion)
NUM : Okay.. Is there any left of recommendations?
Round team : Nothing
NUM : Okay, Good… please do all the interventions that have been planned and
recommended earlier each shift . If there’s any obstacles please may be
reported to me or to the relevant team ..
Nurses : Yes ners.
NUM : I close the nursing round this morning, hopefully what we plan together
can be useful to solve the patient’s problems. Thank you very much for the
