Requirements of Petition for Reconstitution of Title
(Office and Owner’s Copy of the Title Lost)
1. Signed copy of the Petition
2. Tax Declaration (Latest) 3. Tax Clearance (Latest) 4. Certification from the Register of Deeds concerned that the original of the certificate/s of title in the Registry was either lots or destroyed 5. Original with duplicate copies and Xerox of the original technical description of the parcel of land covered by the lost/destroyed certificate of title of title, certified by the authorized officer of the LRA/LMB 6. Certified copy of lot data or area computation prepared by a duly licensed Geodetic Engineer 7. Sepia film plan with print copies of the subject parcel of land prepared by duly licenses Geodetic Engineer who shall certify thereon that its preparation was made on the basis of a certified technical description

The owner’s duplicate of the title duly certified by the clerk of court 4. The certified copy of the original certificate of the title previously issued by the Registry of Deeds 5. Decree of Registration 6. Certification from the LMB that patent title was issued 7. Original with 2 duplicate copies and a Xerox of the original technical description of the parcel of land covered by the lost/destroyed certificate of title of title, certified by the authorized officer of the LRA/LMB 8. Certified copy of lot data or area computation prepared by a duly licensed Geodetic Engineer
