AFFIDAVIT

I, EDJIER GERENIA MENDOZA, married and a resident of Hinaplanon,

Iligan City, Philippines, after subscribing to an oath, in accordance with law, hereby
depose and say, that:

1. I am a Police Officer 2 and was on duty at PNP Station 01 in Tambo, Iligan

City last August 18, 2016 as Mobile 01 Driver;

2. Part of my duty as Driver is to conduct roving, responding calls that needs

police presence, deploying troops and others, from the start and end of my
tour of duty. During that time, SPO1 Erwin C. Pastor was my mobile crew;

3. While roving at around 10:05 in the evening, we found a suspicious person

roaming around at Purok 08, Brgy. Upper Hinaplanon, Iligan City;

4. Around 10:10PM, I called PO2 Raymond Cuyos through his mobile phone
requiring his presence back to the station because of the need to conduct an
investigation about a suspicious person that we found roaming at
Hinaplanon, Iligan City;

5. Around 10:30 in the evening, SPO1 Pastor and I arrived at Station 01 and
saw PO2 Cuyos there who arrived few minutes earlier than us;

6. We immediately brought the person to be investigated to SPO2 Boyonas for

proper disposition and documentation and left thereafter to continue our
roving and went back at around 11:00PM;

7. Around 11:20PM, I was instructed by SPO2 Boyonas to conduct roving and

blocking as a shooting incident transpired at FAMBAM Convenience Store
and the alleged gunman was possibly headed to our area of responsibility
riding a red Yamaha STX motorcycle;

8. Together with SPO1 Erwin Pastor as Mobile crew Patrol, we hurriedly

conducted the roving to the possible routes the alleged gunman would take;

9. We were already out for 30 minutes but the alleged gunman and motor
vehicle described were nowhere in sight;

10. We then decided to go back to our station and we saw Cuyos inside the
office;

11. I am executing this affidavit to attest to the truth of the foregoing statements
and for the purpose of establishing the fact that I called PO2 Cuyos at around
10:10PM and PO2 Cuyos was inside the station when we brought the
suspicious person to SPO2 Boyonas and saw him in the office when we went
back from roving at around 11:50PM.
