This offer is limited to one rebate per product purchased. Qualifying Models must be purchased between July 1 and September 30, 2010. All forms and requested materials must
be postmarked in one mailing within 60 days of the purchase date (“Rebate Submission Period”). Late submissions WILL NOT be honored. LG Electronics MobileComm, U.S.A.,
