APPLICATION FOR RELEASE ON RECOGNIZANCE

BBB, through counsel and unto this Honorable Court, hereby

applies for release on recognizance pursuant to Section 7 of
Presidential Decree No. 968 and Section 12 of Republic Act No. 10389,
and further states that:

1. In an Order dated December 7, 2018, this Honorable Court gave

due course to his Application for Probation.

2. He is currently serving his sentence at the Iloilo District Jail –

Male Dormitory at Brgy. Nanga, Pototan, Iloilo, pending the
investigation of the Iloilo Province Parole Probation Office.

3. He is unable to post bail or proffer any property acceptable as

sufficient sureties for a bail bond because he is an indigent and
his family not engaged in any lucrative employment. A copy of
the Certificate of Indigency issued by the Municipal Social
Welfare and Development Office of Pavia, Iloilo is hereto
attached as Annex “1” and made an integral part hereof.

4. To support this application, Barangay Kagawad AAA and

Barangay Kagawad CCC, who are responsible members of the
community of Barangay Amparo, Pavia, Iloilo, intend to accept
the custody of BBB and guarantee his appearance whenever
told to do so by the Honorable Court or by the Probation
Officer.
 A copy of the Joint Affidavit of Undertaking of Custodians by
AAA and CCC is hereto attached as Annex “2” and made an
integral part hereof.

WHEREFORE, in view of the foregoing, it is most respectfully

prayed that the Honorable Court grants BBB’s release on
recognizance in the custody of Brgy. Kagawad AAA and/or CCC.

Other reliefs, just and equitable, are likewise prayed for.
