DISCHARGE PLAN

Medication
 Instruct the patient to take P.O. medications on time as prescribed.
 Emphasize the importance of adhering to the prescribed therapeutic regimen.

Environmental/Exercise
 Encourage S.O. to provide patient a well-ventilated area.
 Stress the importance of cleanliness to the patient.

Treatment
 Advise to contact health care provider for any unusualities.
 Instruct the significant others to applied sterile dressing to open wounds to prevent infection.

Health Teaching
 Encourage patient to express feelings to S.O. and support group.
 Emphasize the irritants of patient’s condition.
 Promote adjust of lifestyle and responsibilities to accommodate limitations imposed by fracture.
 Observe signs of mood changes or any unsualities.

Observable cues with the S.O.


 Discuss the signs and symptoms of patient’s condition.
 Instruct to go immediately to the physician if unusualities occur.

Diet
 Diet as tolerated and ordered by the physician.
 Encourage the patient to consume diet to promote bone healing (Calcium, Vit. C and high
protein diet).
 Instruct S.O. to avoid giving chocolates and caffeinated beverages to the patient.
 Increase fluid intake.

Spirituality
 Encourage patient for counselling as for expected interpersonal distress.
 Suggest to attend Sunday service mass in church as to boost spirit and to prevent distress.
