Process for downloading of Health Identity E-Card

(Health Identity Card will help employees in availing Cashless Facilities

in the United India TPA Network Hospitals)

Sum Insured : Hospitalisation & Domiciliary Treatment Coverage

(Officer: Rs.4,00,000/ : Award Staff: Rs.3,00,000/-)

Every employee is requested to take a print-out of the Health Identity E-Card of

him / her and also for their dependants who are under cover of Bank’s Medical
Insurance Scheme w.e.f.01-10-2015.

Step : 1 :
Please go to United India TPA website - http://www.heritagehealthtpa.com

Click on “IBA BANK STAFF”

Step : 2 :

On clicking “IBA BANK STAFF”, System will show User the below mentioned
screen. Click on “E-CARD”

On clicking “E-CARD”, System will show User the below mentioned screen.
Select Bank from the drop down box – “UNITED BANK OF INDIA” and

Put SPF Number in EmpID column. (Card Number is not necessary for
generation of E-Card)

Step : 4 :
After selecting “United Bank of India” (FOR EXISTING EMPLOYEE) and “United
Bank of India-Retired” (for retired employees) from drop down box and on
putting SPF No. in EmpID column and On Clicking to “Search” button (just
below “EmpID” field) the System will show User the below mentioned screen

Take a print out of all the Health Identity E-Card of you and your
dependants. Try to carry the same with you for any unforeseen incident.
