Respectfully returned through the school head of Aguinaldo National High School
the herein APPROVED request of BERNICE B. BUNHIAN to enroll and continue her post
graduate studies this summer SY 2018-2019 at the Ifugao State University, Potia,
Alfonso Lista, Ifugao on the following conditions:

a. That no government time and/or resources including computers shall be

used for her studies;
b. That should there be any conflict between her official functions and her
studies, her official duties shall prevail;
c. That no classes or office hours be sacrificed. She will not leave her post on
Fridays to prepare for her attendance in school;
d. That the maximum load/unit to be enrolled should not exceed nine (9)
units per semester and twelve (12) units during summer if the school/
institution she enrolled in allows;
e. That the study applied for I related to her position;
f. That a certified photocopy of her Official Transcript of Records be
submitted to the Division Office upon completing her degree.

This study permit is valid until course applied for is finished, provided that the
Division Office approval is sought one month before enrollment.

Violation of any of the above conditions shall cause the CANCELLATION of this
permit.
