AFFIDAVIT OF DISCREPANCY
I, LINA P. TOLEDO, of legal age, Filipino Citizen and residing at No. 8 Macopa Street,
Phase 1-D Valley View Executive Village, Brgy. Muntindilaw, Antipolo City, after having
been duly sworn to in accordance with law, hereby depose and say:

1. That I am the natural mother of ALEXANDER DAVID T. LINCO who was born
on January 10, 1990 at Quezon City, Metro, Manila and my said son was duly
baptized/ christened at St. Francis of Assisi Parish. Greenhills. Metro. Manila
on April 15, 1990 by the named ALEXANDER DAVID LINCO, photocopy of his
certificate of baptism is here to attached as Annex”A” and made an integral
part hereof:

2. That since my said son started his primary schooling until he enrolled and
study at his secondary/high school curriculum. He used his first name
ALEXANDER DAVID based in his said certificate of baptism ( Annex “A” hereof)
and the same first name appeared in all his school records/documents and
all transactions;

3. That however, when I secured an authenticated copy of his certificate of Live

Birth with the National Statistic Office (NSO). I found out and discovered for
the first time that his first name was printed and recorded only thereat as
ALEXANDER (without the name DAVID)., copy of which is hereto attached as
Annex “B” and made an integral part hereof;

4. That ALEXANDER T. LINCO is my son’s actual and correct FULL NAME as

registered in his certificate of Live Birth (Annex “B” hereof)

5. That I am executing this affidavit to attest to the truth of the veracity of thye
forgoing facts and purposely to establish that the name ALEXANDER DAVID T.
LINCO and ALEXANDER T. LINCO . belong/refer to one and the same person
and for all legal intents that this affidavit may serve.
