AFFIDAVIT OF NON-EMPLOYMENT

I, MARLOU JONES R. APOG, 19 years of age, single, and a

resident of Brgy. Bahao, Tago, Surigao del Sur, under oath, hereby
depose and say:

1. That I am a graduating student of Surigao del Sur State

University-Main Campus, Tandag City, Surigao del Sur;

2. That as of the present time, I am not employed in any capacity

at any corporation, company, and individual person,
considering that I am a full time student;

3. That I have been unproductive, not gainfully employed and

solely dependent upon the support of my parents;

4. That I am executing this Affidavit of Non-Employment to

attest to the truth of the foregoing and to support my
Application for Petition for Correction in the Entry of SEX in
my Certificate of Live Birth.
