Letter of consent to participate in the study

Dear participant
You are invited to participate in a survey of student opinion about the studies of smoking
cessation and non-communicable diseases by pharmacy students in ASEAN. Our goal is to
collect and analyze data from the final year ASEAN pharmacy students to determine the opinions
of the students on teaching activities and the learning process in pharmacy courses regarding
smoking cessation and non-communicable diseases.
If you decide to participate, please complete the enclosed survey. Your return of this survey is
implied consent. The survey is designed to determine the opinion on teaching and learning about
smoking cessation and non-communicable diseases, and to the relationship between the course
of study, assessment and practice of knowledge, skills, and attitudes. We expect it will take 10-
15 minutes to complete the survey. There will be no benefit to you for answering the survey, but
your responses will be used to synthesize the information for the development of the pharmacy
curriculum. Any discomfort or inconvenience to you derives only from the amount of time taken to
complete the survey. We appreciate your cooperation.
Any information that is obtained in connection with this study and that can be identified with you
will remain confidential and will not be disclosed. Your decision whether or not to participate will
not prejudice your future relationships with Mahasarakham University. If you decide to
participate, you are free to discontinue participation at any time without prejudice.
