Received is the body of a well-nourished adolescent male appearing compatible with the reported age. There are
marfanoid features. The body is clad in underwear. No valuables accompany the body. Identifying marks consist of
scars as diagrammed. Evidence of medical intervention includes defibrillator pads and ECG leads.

INJURIES
There are no significant internal or external injuries.

INTERNAL EXAMINATION
Body Cavities
The bilateral pleural and peritoneal cavities contain no significant fluid. No adhesions are present. The organs are in
their normal anatomic positions.
Cardiovascular System
Heart Weight 460 grams
The pericardial sac is free of significant fluid and adhesions. The coronary arteries arise normally, follow the usual
distribution and are widely patent without evidence of significant atherosclerosis or thrombosis. The chambers and
valves bear the usual size-position relationships and are unremarkable. The myocardium shows no evidence of acute
infarction, scarring or focal lesions. The aorta and its major branches are intact without significant atherosclerosis.
Respiratory System
Right Lung Weight 950 grams
Left Lung Weight 980 grams
Examination of the soft tissues of the neck including the strap muscles and large vessels reveals no abnormalities. The
hyoid bone and laryngeal cartilages are intact. The larynx is clear. The upper and lower airways are free of debris and
foreign material. The lungs are normally formed. The parenchyma of both lungs is firm and congested without obvious
consolidation or focal lesions. The pulmonary arteries are free of thrombi or emboli.
Gastrointestinal System
The gastrointestinal tract is intact throughout its length and is unremarkable. The stomach contains 200 ml of partially
digested food consistent with chicken.
Liver
Liver Weight 2110 grams
The capsule is intact and the parenchyma is unremarkable. The gallbladder contains bile. The extrahepatic biliary tree
is patent.
Spleen
Spleen Weight 260 grams
The spleen is normally formed; no focal lesions are present.
Pancreas
The pancreas is of normal size, shape and consistency without focal lesions.
Urinary
Right Kidney Weight 220 grams
Left Kidney Weight 240 grams
The kidneys are of normal size and shape. The capsules strip with ease from the underlying smooth cortical surfaces.
The renal architecture is intact without focal lesions. The bladder contains urine.
Reproductive
Grossly unremarkable.
Endocrine
The thyroid gland and bilateral adrenal glands are grossly unremarkable.
Neurologic
Brain Weight 1710 grams
Reflection of the scalp reveals no evidence of injury. There are no skull fractures identified. The leptomeninges are
thin, delicate and congested. The cerebral hemispheres are swollen. The vasculature at the base of the brain is intact
without significant atherosclerosis. Coronal sections reveal normal architecture without focal lesions.

Musculoskeletal System
Grossly unremarkable; no fractures are identified.

MICROSCOPIC EXAMINATION
Microscopic Comment
Section of brain shows perivascular extravasation of red cells and scattered foci of perivascular hemosiderin.

Sections of lungs show edema, mild acute bronchitis and chronic reactive airway changes.

Section of liver shows peri-portal hepatocyte nuclear inclusions.

Sections of heart, kidney, pancreas are non-contributory.

SUMMARY AND INTERPRETATION


The decedent is a 16-year-old male who was found unresponsive in bed by next-of-kin. He has no significant reported
medical history. He is a competitive swimmer and otherwise healthy person. There are no reports of drug abuse.

Autopsy examination reveals an enlarged heart without the features of hypertrophic cardiomyopathy, mild bronchitis,
and pulmonary edema. Visual inspection reveals some marfanoid features but is not diagnostic for the syndrome.

Toxicology testing is non-contributory.

Based on the history and autopsy findings, it is my opinion that the cause of death in this case remains undetermined.
Injury and obvious natural disease that can result in sudden unexpected death have been excluded. The remaining
etiologies that cannot be excluded through gross, microscopic, and toxicological examination are almost exclusively
related to cardiac function and include, but are not limited to cardiac channelopathies. A tube of blood was obtained at
autopsy and is preserved if further genetic testing is pursued.
