PETITION

PETITIONER, LUCIO ORIO TAN, JR., unto this Honorable Court,

most respectfully states that:

1. He is a Filipino citizen, over twenty one years of age,

and a resident in the Philippines since birth and with office
address at Barangay Poblacion, Municipality of Sibonga, Province of
Cebu where he also maintains and holds his private law office;

2. He has satisfactorily completed the required course in law at the

University of the Visayas, Cebu City and has earned the degree of
Bachelor of Laws (LLB.) in March 21, 1984;

3. He has passed the Bar Examinations and admitted to practice

law in the Philippines on May 21, 1999 with Roll of Attorneys No. 44656.

issued by the 0ffice of the Bar Confidant, Supreme Court of the

Philippines, is herewith attached as Annex "A" and made an integral
part of this petition;

4. He is a member of the Philippine Bar in good standing with

Clearances from the Office of the Bar Confidant of the Supreme Court
and the Integrated Bar of the Philippines. Copy of said clearances are
herewith attached as Annex "B" and Annex "C", respectively, and made
integral parts of this petition;

5. The undersigned has not been convicted in the first instance of

any crime involving moral turpitude, nor charged in any
administrative case in connection with his present profession.
Attached herewith as Annex "D" to form part of this petition is the
Clearance from the National Bureau of Investigation (NBI);

6. Petitioner is a registered member of the Integrated Bar of

the Philippines, Zamboanga del Sur Chapter with office at 2nd Floor
Bulwagan Ng Katarungan, City Hall Compound, Pagadian City, having
paid his IBP dues and cleared of any and all arrears, as supported by a
copy of his Certification, duly issued by two (2) Executive Officers,
namely, the IBP Chapter President and IBP Chapter Treasurer,
respectively, hereto attached as Annex "E" and made an integral part
of this petition;

7. Petitioner has likewise paid the required Membership Dues up

to CY 2019 and Building Fund contribution to the Integrated Bar of the
Philippines, as supported by a copy of his Certification, duly issued by
the National Treasurer of the Integrated Bar of the Philippines with

National Office at 15 J. Vargas Avenue, Ortigas Center, Pasig City, hereto

attached as Annex “F” and made an integral part of this petition;

8. Petitioner has also paid his Professional Tax Receipt for the
current year under 0fficial Receipt No. 1640193 dated December 6,
2018 at Cebu City. A copy of the receipt is attached as Annex "G" and
made an integral part of this petition;

9. Petitioner has not yet applied nor been commissioned as

Notary Public in any other jurisdiction. He, therefore, desires to be
commissioned as Notary Public for and in the Municipalities of Sibonga,
Dalaguete, Alcoy and Boljoon, all of the Province of Cebu, and
possesses all the qualifications and none of the disqualifications for said
office;

10. Petitioner will submit his notarial books and acknowledged

documents after the expiration of his notarial commission as
provided for by law;

11. Petitioner has read, understood and is willing to abide with

the 2004 Rules on Notarial Practice.

12. Petitioner is attaching as Annex "H", a facsimile of his notarial

seal prepared in accordance with Section 2, Rule 7 of the 2004
Rules on Notarial Practice for approval of this Honorable Court,
together with 3 specimens of his official signature.

13. Also submitted to form integral parts of this petition are

petitioner’s three (3) passport sized colored photographs with light

petition.

PRAYER

WHEREFORE, premises considered, it is most respectfully prayed

of this Honorable Court that:

1. The Petition for Commission as Notary Public filed by the

undersigned be acted favorably; and

2. Petitioner be commissioned as a Notary Public in the

Municipalities of Sibonga, Dalaguete, Alcoy and Boljoon, all of the
Province of Cebu, which are within the territorial jurisdiction of this
Honorable Court.

1. I am the petitioner in the above-entitled petition;

2. I have caused the preparation of the foregoing petition;
