With reference to the attached Memorandum of GILBERTO S. REYES, Asst. Secretary for
Technical Services dated September 17, 2018 to Director WILFREDO S. MALLARI. regarding
the findings/noted defects on the Quality Assurance Unit’s Project Assessment Report
(Report Reference No. QAU-IV-B-18-JUL), you are hereby directed to undertake the
following at your own expense:

1. Clean the joint then apply sufficient asphalt sealant on the construction joints of the
completed concrete pavement at block 1 to 25 all at left lane;
2. Strictly monitor the PCCP blocks 40 to 45 at left lane with noted rain damaged
surface and if further deterioration occurs within the defects liability period, remove
and replace the affected blocks.

Take pictures before, during and after rectification of the above noted defects as evidence
of your action taken.

1. Seal with pressurized concrete epoxy of corner crack at block 98 right side of Section
2 and Transverse cracks located at middle third of block 38 and 57 right lane of
Section 2;

2. Strictly monitor the PCCP blocks 52 and 59 at left lane with noted minor scaling and
Temperature cracks at block 53 left lane of Section 1 and blocks 1 to 5, 14 to 17, 22
to 30 and 85 at right lane of Section 2, if further deterioration occurs within the
defects liability period, remove and replace the affected blocks; and

3. Patch the noted footmarks with epoxy mortar at blocks 29, 33, 53, 64, and 65 at
right lane of Section 2.

Take pictures before, during and after rectification of the above noted defects as evidence
of your action taken.
