Johann was generally alert and conscious during the evaluation.

He had no stranger and


separation anxiety as he went with the therapist immediately inside the cubicle when asked to
by the undersigned. However, he was noted to have difficulty maintaining eye contact when
being talked to.
Johann was given a couple of table top activities to work on. He was asked to complete 6 pcs.
Jigsaw puzzle, fine motor tasks and pre-writing and writing activities. He was generally
uncooperative during the evaluation as he needed repeated prompts to do and follow a single
task. He also tends to escape from tasks and tried to run out of the cubicle a couple of times. It
was also observed that he was very distracted with visual and auditory stimuli. He kept on
looking at other people outside the cubicle and stops doing a task when he hear noise from the
other cubicles in the center. Frequent fidgeting was also observed during the session

Johann was given a 6pcs. Puzzle task and 2-d tile design to copy. He showed difficulty finishing
these tasks on his own even when he is using trial and error and needed cues from the
undersigned to complete both activities. He was also asked to copy his name. He used his R
hand and tripod grasp on pencil. His handwriting appearded to be readable but did not write
within the borders given and also did not follow correct letter formation of letters.
Sensory Processing

Based from the questionnaires and unstructured clinical observation, Ethan showed typical
performances in the following sensory areas:
 Visual Processing
 Sensory Processing related to endurance/tone

On the other hand, problems in processing sensation in the following areas has been reported
and observed:

Balance and motion or the Vestibular sense orients us to gravity. Our head moves in response
to the changes in center of gravity to maintain balance and equilibrium. Disorder in processing vestibular
input may result in poor balance, poor postural control and problems with the perception of body
movements in space. At the same time, this part of the questionnaire measures the Body awareness or
proprioception which is sensed from the joints and muscles. This allows us to move in space and around
us without bumping or crushing into other people or furniture. This sensory system also enables us to
move confidently and spontaneously with safety and in timely manner. Based from the questionnaire,
probable difference more than others has been reported by the parents. Johann showed vestibular and
proprioceptive seeking behaviors such as seeking all kind of movement activities that sometimes
interferes with daily routine (i.e. fidgeting on seat, etc). During the evaluation, it was noted that Johann
is hyperactive and had difficulty sitting still. Rocking himself on his chair was also observed.

Multisensory processing is the regulation of more than one sensation simultaneously to provide
a more adaptive response to the environment. Result given by the parent suggests a definite difference
more than others. He was observed to have difficulty paying attention, gets distracted easily, etc. It was
observed during the evaluation that Johann frequently looks away from tasks to look around the center.

Oral Sensory processing is regulation of sensory inputs inside and around the mouth as well as
on the perioral area. It measures the child’s response to tactile and proprioceptive sensations in and out
of the oral area in addition to gustatory and olfactory stimuli. Johann scored definite differences more
than others based from the questionnaire answered by his parents. It was informed that Johann is quite
picky eater, especially regarding food textures, temperature and tastes.

Auditory Processing is a natural process of taking in sound through the ear and having it travel to the
language area of the brain to be interpreted. When a child is having trouble with auditory processing, his
or her ears can pick up the sounds, words, etc., but the brain is not able to process the sounds properly.
Therefore, the auditory information becomes jumbled up or confused and becomes misunderstood.
Johann scored Definite Difference more than others. It was informed that Johann was having difficulty
following verbal instructions and is having a hard time answering simple questions.

The tactile system includes nerves under the skin’s surface that
send information to the brain. This information includes light touch, pain, temperature,
and pressure. These play an important role in perceiving the environment as well as
protective reactions. When the tactile system is immature and working
improperly, abnormal neural signals are sent to the brain which can interfere with other
brain processes. This, in turn, causes the brain to be overly stimulated and may lead to
excessive brain activity, which can neither be turned off nor organized. This type of over-
stimulation in the brain can make it difficult for an individual to organize one’s behavior
and concentrate and may lead to a negative emotional response to touch sensations. Johann scored
Probable difference more than others in the questionnaire. It was reported that Johann avoids his hands
getting messy but likes touching people and objects with certain textures and surfaces.

These areas are most likely to provide challenges for Johann. When children have difficulty in a
sensory system, it means that this form of sensory input is confusing, upsetting, or not meaningful to the
child. In any case, difficulty with sensory input can interfere with Ethan's ability to complete important
activities as successfully as other children do.

Modulation

Johann has difficulty in the following sections based from the results of the questionnaires:
 Modulation of Sensory Input Affecting Emotional Responses
o Johann could not recognize facial expressions and body language. Hence, he continues to
do unexpected behaviors even when his parents are already firm to him.
 Modulation Related to Body Position and Movement
o Johann seems to be accident-prone especially when playing where he takes excessive
risks e.g. moving and climbing that compromises safety.
 Modulation of Movement Affecting Activity Level
o Johann showed problem with remaining seated for long periods, seemed to be always
alert or “on the go” and easily becomes excitable during movement activities.
 Modulation of Visual Input affecting emotional responses and activity level
o Johann seems to notice and gets distracted with other people inside the room. It was
reported that he likes to stare intensively at objects and people.

Behavior and Emotional Responses

Johann has difficulty in the following sections based from the results of the questionnaire:

o Johann usually takes time or refuse to finish tasks that he does not like
 Indicating Thresholds for Response
o Johann likes to jump from one toy/ activity to another.
 Emotional/ Social Responses
o Johann is usually uncooperative and stubborn to his parents and in school. He
occasionally throws temper tantrums when he does not get what he wants.

Quadrant Summary

Based from the questionnaires and unstructured clinical observation, Johsnn doesn’t show any problem
with low registration. However, he showed sensory seeking, sensitivity and avoiding patterns that may
interfere with his daily routine.
