THIS IS TO CERTIFY that I, Imelda Guillermo, MD, FPPS, Chief of Hospital / PHO 1 , of Gov.
Faustino N. Dy Sr. Memorial Hospital , Ilagan , Isabela do hereby accept from the Cagayan Valley Center
for Health Development the above-prescribed Health Facility funded under the Health Facilities
Enhancement Program and implemented by the CVCHD under the following conditions:

 That this Certificate of Acceptance applies only to the completed portions of the facility which are
included in the Program of Works prepared by the CVCHD and which where jointly inspected and
certified completed by the concerned parties;
 That the Provincial Government of Isabela – Gov. Faustino N. Dy Sr. Memorial Hospital accepts
the facility with the assumption that it was constructed with the highest quality standards of the
DOH thereby assuring it’s structural integrity,
 That the Gov. Faustino N. Dy Sr. Memorial Hospital, owned and operated under the Provincial
Government of Isabela assumes full responsibility for the proper maintenance and operation of
the completed facility, and;
 That this Certificate of Acceptance does not relieve the Contractor from the enforcement and
implementation of all legal matters pertaining to the contract of construction and applicable
provisions of Republic Act 9184.
