FALL’S TRAJECTORY

Ringkasan Kejadian:

Pada tanggal 28 Juni 2011, sekitar pukul 17.30 seorang karyawan pihak ketiga terjatuh dari ketinggian 20
meter yang mengakibatkan korban tewas. Korban bekerja sebagai helper yang membawa pipa untuk system
udara bertekanan di sepanjang stuktur belt conveyor (panjang 100 m dengan kemiringan 15%) yang sedang
dibangun. Menurut saksi mata, setelah berjalan naik menyusuri samping belt conveyor, korban duduk diatas
pipa handrail bagian tengah untuk istirahat, kemudian korban terjungkal kebelakang dan jatuh melalui bagian
yang terbuka (81 cm) di antara pipa pegangan bagian atas dan bawah. Korban memakai alat pelindung diri
lengkap, tali life line dan titik kait yang diperuntukkan untuk pekerjaan proyek ini juga tersedia.

INTI PEMBELAJARAN

1. Pada saat melakukan pekerjaan diketinggian, alat pelindung jatuh harus dipakai dengan benar dengan
hook dikaitkan ke titik kait, silahkan lihat FPE1 working at heights.
2. Jangan pernah gunakan besi pegangan (handrail) sebagai tempat istirahat (duduk).
3. Job safety analysis (JSA) harus jua mempertimbangkan material yang harus disediakan di permukaan
yang lebih tinggi.

FALL’S TRAJECTORY

Summary of Case:

On June 28, 2011, at approximately 17h30 and employee of a third party service provider, suffered a fall from
about 20m high, resulting in his death. The victim was working as a helper carrying pipes for a compressed air
system along a conveyor belt structure (100m long with 15% slope) under construction. According to an
eyewitness right after walking up, for resting purposes the victim sat on an intermediate handrail’s tube and fell
backwards through an opening (81 cm) between the bottom and top rail tubes. The person was wearing his full
personal protective equipment, including harnesses and life lines corresponding to the task that was being
performed and anchoring points were available at this point.

KEY LEARNING POINT

1. When conducting working at heights, fall protection equipment needs to be used properly by anchoring
hook to designated anchorage point, please refer to FPE1 working at heights.
2. Never use handrails as a place to rest (sitting).
3. Job safety analysis must also consider materials’ supplies to elevated working sites.
