1. There was one time in our laboratory wherein i was posted in the receiving/out patient
department.An out patient gave his request and his specimen(blood chem) was then received into the
laboratory. But everyone was preoccupied with their own tasks. Everyone in the section also wasnt
paying attention anymore. It was a typical "toxic" duty.Apparently, upon further notice of the chemistry
section, his request did not contain the trodat and sign of the physician.On top of this, his specimen was
hemolyzed and needed reextraction.

2. The problem was realized among the interns of the section. They did not pay attention to the
completeness of the request. They also shouldnt have let the out patient leave without making his
specimen was sufficient for testing.

3.The staff in charge was notified of the problem. The name of the patient was then endorsed for
reextraction. His specimen was double checked if there was any hemolysis.The request was sent to his
physician for completion.

4.To avoid further errors of the same nature, the interns are advised to have a presence of mind in
receiving and rejecting specimens. To minimize "toxicity" the interns are suggested to have a decking for
an easy workflow.
