PETITION

Petitioner xxxxxxxxxxxxxxxx, assisted by the undersigned Public

Attorney and before this Honorable Court, most respectfully aver:

1. Petitioner is of legal age, Filipino, widow and a resident

xxxxxxxxxxxxxxxxxxxx. She is represented by the
xxxxxxxxxxxxxxxxxxxxxxxx with office address at
xxxxxxxxxxxxxxxxxxxxx where orders and other legal processes of
this Honorable Court may be served;

2. Petitioner has executed a Special Power of Attorney, appointing

therein xxxxxxxxxxxxxxxxx as her attorney-in-fact, authorizing the
latter to perform every act necessary to secure a second owners
duplicate copy for the Transfer Certificate of Title subject of the
instant petition. An original copy of said Special Power of Attorney
is attached and marked herein as Annex “A to A-1”;

3. Petitioner is the registered owner of a Transfer Certificate of Title No

T-119784(M), a parcel situated in the boundary of Brgy. Banga and
Brgy. Lawa, City of Meycauayan, Bulacan. The True Electronic
Copy of TCT No. T-119784(M) is attached herein and marked as
Annex “D to D-4”;

4. The duplicate owner's copy of the title of this parcel of land was,
however, lost and is now beyond recovery, the circumstances of
 which are alleged in an Affidavit of Loss executed by herein
Petitioner. The original copy of the said Affidavit of Loss is attached
and marked herein as Annex “E”;

5. Prior to its loss, the subject Title has not been mortgaged or
subjected to any encumbrance;

6. The Affidavit of Loss executed by herein Petitioner was annotated to

the original copy of TCT No. xxxxxxxxxxxxx evidenced by the
encumbrance annotated in the fifth page of Annex E-4 with Entry
No.: 2018026681 dated October 12, 2018, which is hereby bracketed
and marked as “Annex “F”;

7. The petitioner is willing to comply with the requirements of posting

as required by law.

PRAYER

WHEREFORE, it is most respectfully prayed that, after notice

and hearing, an order be issued directing the Registry of Deeds of
Meycauayan City Bulacan to issue a Second Owner’s Duplicate
Copy of TCT No. xxxxxxxxxxxx in favor of the petitioner.

Other just and equitable reliefs are likewise prayed for.
