HEALTH SAFETY AND ENVIRONMENT PLAN

1. INTRODUCTION

This HSE Plan has been prepared and shall be used by SUBCONTRACTOR during the
construction of PIPING UNDERGROUND, Safety and Environmental (HSE) are the top priorities
for SUBCONTRACTOR. Therefore the SUBCONTRACTOR goal is to meet all the Project
Specification, CONTRACTOR’S HSE requirements and Government Regulations.

SUBCONTRACTOR will make sure that all designs, procurement, construction and commissioning
activities will be carried out according to Project Specification, CONTRACTOR’S HSE Policies,
Standards, and Government Regulation. SUBCONTRACTOR will issue a CSMS
(SUBCONTRACTOR Safety Management System) to ensure their SUBCONTRACTOR’s to meet
the proper HSE regulations.

HSE activities will be audited as part of the management process. This plan describes the overall
organization of the HSE staff, how it interfaces with other disciplines, and how HSE activities are to
be organized and managed.

2. SCOPE

This HSE Plan is designed to assure the save all construction activities and related activities
performed on the job site during Construction, periods at PROJECT.

3. THE HSE OBJECTIVE

The HSE targets from PT. KPM O G during project execution and design are as follow:

a) Zero fatalities for CONSTRACTOR’S personnel, SUBCONTRACT & third parties


b) Develop and maintain a zero incident / accident free procedure
c) Minimize adverse impact from the Project to the near community
d) Make maximum possible benefits to people from the near community
e) Achieve zero recordable loss frequency rate
f) Zero Lost Time Injury Frequency Rate
g) Non permanently disabling injuries or illness
h) Zero road traffic accident for drivers associated with the Project

4. RESPONSIBILITY

4.1 Project Manager

• Project Manager as Top Leadership in the safety committee of the PROJECT


• Implement HSE Policy for the PROJECT
• Ensure that state and local safety laws, regulations, codes and rules are observed
• He has to responsible for review HSE program implementation, HSE performance in
PROJECT

4.2 Site Manager

• He has the responsibility concerning safety to all personnel in PROJECT work place.
• He is authorized to take necessary action against any HSE violation by
SUBCONTRACTOR or SUBCONTRACTOR’s personnel.
• He must carry out the HSE program for the construction of the PROJECT
• Stops any operation or activity that constitutes an imminent hazard to personnel or
could result in loss of equipment or facility and environment.

4.3 Field Supervisor

• Field Supervisor as member of the Safety Committee.


• Assist in pre-planning job safety analysis for all phases of the construction.
• He must execute Safety Evaluation Program before he begins work activities.
• He has the responsibility to the safety concern all Supervisor work activities.
• He has the safety responsibility concerning all activities SUBCONTRACTOR in his area
• He has rightful authority to stop all the unsafe action and unsafe condition in his area,
He must inform the HSE manager or other designated HSE person about the safety
observation result in site.

4.4 Supervisor

• Assist in pre-planning job safety analysis for all phases of construction.


• Assure, encourage, and recommend the use of safe work practices by all employees.
• Assure that supervisor and SUBCONTRACTOR implemented in the site Safety
Program.

• Insist that supervisor and SUBCONTRACTOR give instructions and training in site
operating procedures to assigned personnel and do not require or permit employees to
work in an unsafe manner.
• Review Supervisor's accident reports, taking corrective action as necessary.
• Arrange for and attend Supervisor's weekly "tool-box" safety meetings.
• Conduct safety checks on a daily basis, making on-the spot corrections to conditions
noted to eliminate unsafe conditions or work practices.
• Enforce employee adherence to the Safety program and recommend and/or administer
disciplinary action for violations.
• Be familiar with the requirements of Construction Safety Codes applicable to the
operation and activities.
• He must report to the CONTRACTOR HSE Manager about safety observation result.

4.5 Supervisor

• Assure, encourage, and recommend the use of safe work practices by all employees.
• Assure that the SUBCONTRACTOR are instructed in the Safety Program.
• Insist that SUBCONTRACTOR give instructions and training in site operating
procedures to assigned personnel.
• Supervisor must make accident reports, taking corrective action as necessary.
• Arrange for and attend SUBCONTRACTOR’s weekly "tool-box" safety meetings.
• He must execute safety observation program in all his locations and activities.
• He must report to Supervisor about safety observation result.
• Conduct safety checks on a daily basis, making on-the spot corrections to conditions
noted to eliminate unsafe conditions or work practices.
• Enforce employee adherence to the Safety program and recommend and/or administer
disciplinary action for violations.
• Be familiar with the requirements of Construction Safety Codes applicable to the
operation and activities.

4.6 HSE Coordinator

• Ensure that Government safety laws, regulations, codes and rules are observed
• Assists top management in preparing safety policies and then ensuring that they are
carried out.
• Monitor all activities where accident could occur that would cause injury to personnel,
damage to equipment or facilities, or losses of material
• Halts any operation or activity that constitutes an imminent hazard to personnel or could
result in loss of equipment or facilities and environment.
• Assist in the formation of safety committees and assist them carrying out their activities,
the responsible of safety committee formation is SUBCONTRACTOR Project Manager.

• Establish and monitors programs for detecting hazardous conditions personnel


protective equipment are available, used and in good working order prior to possible
use
• Make certain hazardous areas, entrance and exits, and dangerous equipment are
posted in accordance with prescribed standards
• Conducts safety training of personnel at all levels.
• Approved procedures for hazardous operation.
• Periodically inspects emergency supplies located in the site to ensure that nothing has
been removed without authorization and that they are ready for their intended purposes.
• Maintains all record that are related to safety activities.
• Keeps informed on latest development in activities that relate to the job, such as new
safety equipment; hazardous materials that may be used in the plant; changes in laws,
regulations, codes
and rules; new methods of accident prevention. Responsible to Project Manager for all
safety activity that are performed at plans.

4.7 Safety Inspector

• To conduct schedule HSE meeting, HSE Inspection tours and suggest proper HSE
methods to prevent accident, environment control and reports to Owner and local
Government.
• To conduct and assist HSE induction and training.
• To investigate all kind accident and suggest proper counter measure for preventing
recurrence.
• To make HSE program good coordination between all sections
• Develop and maintain applicable HSE regulation and standards for the project in
accordance with SUBCONTRACTOR accident prevention standards, Owner’s standard,
Government regulation and local HSE standard.
• Promote HSE on the project trough orientation, HSE meeting, reward of project
personnel.

5. SAFETY PLAN

SUBCONTRACTOR to prepare, implement and asses their JSA which minimum required to
following:

5.1 SAFETY PROGRAM

SUBCONTARCTOR is committed to and continuously implements its corporate policy, in all


SUBCONTRACTOR operation worldwide. To achieve this aims, SUBCONTRACTOR has
developed a Safety as follows:

5.1.1 Safety Orientation

Safety orientation / briefing will be conducted to all new comers. Also documentation
shall be a summary of key items given to each person on site when attending
briefing sessions.

The orientation criteria are:


 New comer in this project
 New employees

The minimum material of orientation aren’t limited to following:


 What is this project
 What are hazardous this project
 How to emergency evacuation in this project
 Emergency communication
 What is all material hazard
 Site regulation, Owner regulation.
 Safe practices

5.1.4 Site Inspection (Safety Patrol)

b. Joint safety inspection (weekly). The inspection shall be documented.

5.1.5 Safety Consultation

5.1.6 Safety Performance

SUBCONTRACTOR site should keep a record of safety performance from


time to time Safety performance report will be submitted on a monthly basis

c. SUBCONTRACTOR Site Manager

Report on minor accident (no lost time) shall be submitted weekly to


SUBCONTRACTOR Site Manager In the event of a serious injury it shall be reported
immediately to CONTRACTORS (KPM O.G).

5.2 TRAINING

SUBCONTRACTOR shall ensure HSE training / briefing for employees, vendor,


SUBCONTRACTOR, CONTRACTOR before their start of works.

The minimum material for training are:


• Security and Safety Training
• Job Safety Analysis Training
• Permit to work system
• Confined Space
• Accident Reporting and Investigation
• Basic Fire Fighting
• First Aid Training
• Waste Management
• Personal Protective Equipment

These training for SUBCONTRACTOR G/S, S/V, Foreman at site, and tentative
schedule as required at site.

5.3. DOCUMENTATION

SUBCONTRACTOR will document all construction HSE activities, which is specified


in the documentation system below :

5.3.1 Documentation System

All HSE issues will be documented and the records maintained at the job site.
All documentation and records should be:
• Legible
• Identifiable / retrievable
• Stored and maintained to avoid loss or damage

Documentation and records will include:


• Environmental and injury incident and investigation report
• Loss prevention records
• Equipment inspection records
• Non-conformance and corrective actions
• Training record
• Induction record
• Monthly HSE Report
• Audit Report

The following statistical records will be maintained and recorded on a daily basis
and will be documented in the written minutes from the weekly review meetings:
• Man hours
• Weather
• Record able Cases
• Fatality
• Environment Accident
• Near misses
• First Aid cases
• Equipment and Property damage
• Fire case
• Vehicle accidents

The Project Manager, Site Manager and HSE Coordinator will maintain statistical
data in the form of HSE summary.
A HSE hazard shall be maintained to be followed up on the HSE findings,
recommendations and tasks identified during the project. Unresolved issues of

particular concern shall be reported to the HSE manager, who will initiate corrective
actions to resolve the issues. A copy of this statistical data will be submitted to
CONTRACTORS and SUBCONTRACTOR Home Office on a monthly basis.

5.3.2 Government Requirements

PT.DEC Project Manager will ensure that a legislation register, which contains HSE
issues, will be carefully maintained during the
project. to maintain record keeping will be HSE Manager Responsibility.

5.4 CSMS (CONTRACTOR Safety Management System)

CONTRACTOR safety management system is designed to improve


SUBCONTRACTOR and SUBCONTRACTOR health, safety and environmental
(HSE) performance.

The purposes of CSMS are:


 Improve workplace HSE performance by assisting CONTRACTOR and
SUBCONTRACTOR activities.
 Assist CONTRACTORs in administering programs which are consistent with
SUBCONTRACTOR expectations
 Facilitate the interface of CONTRACTOR’s activities with SUBCONTRACTOR.
This CSMS should protect both CONTRACTOR and SUBCONTRACTOR personnel
from workplace injuries and illnesses.

After the SUBCONTRACTOR is awarded, SUBCONTRACTOR will execute pre-job


activities before contract as per the following chart execution.

5.5 MANAGEMENT OF DANGEROUS MATERIAL

5.5.1 Transport and Storage

Dangerous material should not be stored or carried with other dangerous materials
where they may interact in such a way so to increasing the danger to persons,
property or the environment. SUBCONTRACTOR will cooperate with and follow
government regulation, if there are any hazardous material in transportation and
storage during the project execution.

5.5.2 MSDS (Material Safety Data Sheet)

A material safety data Sheet is a document that provides information on the


properties of a substance so that a user may understand and minimize risks in the
use of the chemical in normal operations and may implement appropriate
procedures in an emergency. SUBCONTRACTOR will ask chemical suppliers to
provide their MSDS. Moreover, SUBCONTRACTOR will provide MSDS in project
site.

SUBCONTRACTOR will documented MSDS in Warehouse.

5.6 REPORT AND INVESTIGATION

5.6.1 Accident and Incident Reports

Accident reports, investigations, analysis and other record keeping tasks are
elements of the construction in loss prevention program.
Below is a summary of reports, records and other analysis that are
SUBCONTRACTOR’S job during construction:
• Accident report
• Accident investigation / witness statement
• Medical records and Treatment report
• Damage record
• Insurance record
• Monthly safety performance report
• Safety Training
• HSE observation
Each of these reports satisfy one or more of the following requirements:
• The inspection reports and monitoring indicate the current status of the job
site environment, equipment & procedures and any necessary remedial
action
• Accident reports, investigations, medical records, damage records and
summaries can indicate the number, extent, trends, cause and costs of
“unplanned event” and accidents.
• HSE meeting notes, etc are indicators of SUBCONTRACTOR’s compliance
with national & local Laws or with CONTRACTORS requirements.

SUBCONTRACTOR will ensure that all reports are accurate, coherent, verifiable,
and issued promptly.

5.6.2 Incident Investigation

Incident resulting in injuries or losses or near misses discovered are investigated,


documented by the safety supervisor and reviewed by the construction manager

and the HSE manager. The purpose / aim of the investigations are to uncover root
causes of the HSE problems and to change or modify the existing circumstance,
condition or act. There will be an interface meeting with KPM O.G each week
following the SUBCONTRACTOR weekly review meeting.

5.7 HSE MEETINGS

Health, Safety and Environmental regularly scheduled meetings are essential


elements of HSE management in safety, health and environmental improvement
during project execution. Weekly HSE

meeting will be held under the chairmanship of the Contractor Project Manager and
attended by relevant personnel, HSE manager and SUBCONTRACTOR .

Other HSE meeting will be scheduled as follows:


• Daily Tool Box meetings, which will be held by HSE Coordinator, Line Supervisors
with their construction labor force prior to work commencement to disseminate
HSE strategies from the review.
• Daily HSE induction sessions for all new employees, SUBCONTRACTOR
employees.
• Weekly management meeting to review past week activities and the planned
activities for the next upcoming week
• Monthly HSE Report form Project Manager will be reported to CONTRACTORS.
• HSE daily walkabouts of jobsite, site accommodation & catering.
• Daily inspection of health facilities and health documentation report. The purpose
of the meetings are to assess the overall effectiveness of the HSE plan and
address the Project HSE trends and implement corrective action as appropriate

5.8 LOSS PREVENTION

SUBCONTRACTOR continuously review site condition, planned construction work,


and other site activities by all personnel, Hazardous operations, etc are planned and
executed in accordance with CONTRACTOR’S HSE policies.

6. SITE HEALTH PLAN

The HSE Coordinator is responsible for all health matters. Medical services such as first aid
for all SUBCONTRACTOR’s employees will be provided by a contract medical service.

SUBCONTRACTOR will maintain the hazard register, originally developed during


the design phase, as required by the project specification, which include health
hazards. All known potential hazards to the health of personnel during the execution
of the work shall be included in this document. Measures to eliminate or control
these hazards are identified in the register, and the register also highlights critical
areas during the performance of work.

6.2 Medical Facilities

The HSE manager is responsible for all site medical and industrial hygiene matters.
SUBCONTRACTOR will have first aid equipment and. Those will be managed
during all working hours.

SUBCONTRACTOR will joint with CONTRACTORS in emergency response plan,


hospital and ambulance facility throughout the project. SUBCONTRACTOR will
inspect personal accommodations for personnel , which will be provided to their
employees by a doctor and HSE line supervisor.

6.3 Biological Hazards

• Drinking Water.
SUBCONTRACTOR will provide clean drinking water which will be tested to
meet Government Standards.
• Toilets
Provision of toilet will be according to the number of workers.
SUBCONTRACTOR ensure that the facilities are adequate and located at the
work site at proper places.
The toilets are of the portable type, suitable for the construction environment
and will be maintained periodically to its hygienic condition.
• Meal Area and Facility
SUBCONTRACTOR will provide a covered area for its workers and for
SUBCONTRACTOR’s workforce to use for consuming monitoring, lunch and
evening meals, Workers shall not be allowed to remain in the construction area
to consume meals or rest during breaks. No food or drinks or plastic bags
should be allowed to be brought into the work area. Waste water and solid
waste will be properly collected and disposed in permitted areas.
• Alcohol and Narcotics
No alcoholic beverages, narcotics, mind altering drugs or similar substances
are to be brought into the Site or remain at the WORK
SITE.SUBCONTRACTOR personnel and those of its lower tier
SUBCONTRACTOR under the influence of alcohol, narcotics, and mind –
altering drugs shall not be permitted to enter the WORK SITE for any reason.
SUBCONTRACTOR personnel who disregard this requirement will be
terminated from the PROJECT.

• Use of Asbestos Product


SUBCONTRACTOR shall not bring into SITE or any asbestos-based products
for any reason. Should SUBCONTRACTOR be found using asbestos based
products such as fire retardant blanket,
all such material shall be removed from the SITE and disposed off in
accordance with Recognized or statutory asbestos handling procedure at
SUBCONTRACTOR cost.

7. ENVIRONMENTAL PLAN

SUBCONTRACTOR will ask a copy of environmental impact assessment report from


CONTRACTORS and will comply with the requirements of the report as well as
CONTRACTORS, Government & Local environmental standards, Laws and Regulations.

The HSE Coordinator is responsible for all environmental matters. Moreover,


SUBCONTRACTOR will maintain environmental requirements, which applicable to
SUBCONTRACTOR. SUBCONTRACTOR will inspect and audited on SUBCONTRACTOR
performance related to environmental matters.
The measures that SUBCONTRACTOR will take to avoid environmental impact are:

• Doing consultation and participation process with local communities and the
CONTRACTOR’S organization that will allow for the identification of issues significant to
the affected population.
• SUBCONTRACTOR to measure control on the drainage, erosion and sedimentation to
protect water resources.
• SUBCONTRACTOR to execute occupational Health and Safety program.
• SUBCONTRACTOR to control sanitation, waste disposal management, housing, food
and water supply and workplace safety procedures.
• Use of sea front and land will be controlled during construction
• Washing of vehicle tires wash rough before driving on public roadways to prevent dirt.

7.1 Environment Impact Assessment

• Environmental Hazard

SUBCONTRACTOR will maintain avoiding environmental hazard during


construction SUBCONTRACTOR will repair and add to the infrastructure to improve
access to the site and to mitigate increases in local traffic caused by the project
during construction.Air emissions will be minimized by regularly maintaining the
equipment, including regularly scheduled calibration of the fuel injection pumps and
other parts of motorized equipment that impact emissions.

Spillage of fine of raw material from the conveyor and elevator system is
inevitable when transferring solid materials from process unit to another. The
spillage of solid waste materials that are improperly managed would cause
blockages to the plant drainage system and eventually to the main
CONTRACTOR drain outside the plant.

• Mitigating
All observable spillage of solid raw materials and finished product should be
recovered manually and by mechanical shovel and recycled in order to minimize
waste. Screen will be installed at appropriate locations to prevent any raw or
product materials to prevent being discharged to CONTRACTORS drain.

a) Socio-economy

Potential Impacts

• Employment will be temporary enhanced because of the temporary jobs


created during the construction phase, which also means that more
commercial activities will be generated.

• As more people will be employed, housing requirement, utilities and


amenities for the workers are socio-economic aspects that must be attended
to if job satisfaction and productivity are to be sustained. The provision of
these basic needs may create local resentment if the locals are made to feel
deprived by the ensuing situation.

• Mitigating Measures
Local people should be given the opportunity to participate in the
construction activities by awarding them certain suitable at works. Such a
move will help the local population socio economically and reduce potential
hostility.

• As immediate accommodation may be needed to house the workers, the


project proponent should ensure that temporary housing is provided. Renting
available vacant houses in the area can help enhance the local housing
market and foster local goodwill. Special steps must be taken to ensure
reliable and adequate provisions of utilities such as clean water and
electricity for the workers daily use at the temporary accommodations
Sewage and garbage disposals must also be taken care of. Aesthetics and
Culture

• The coming in workers who are expected to be of various ethnic groups may
alter the community structure and as such may affect the sense of
community. However, the impact is expected to be minimal.

• A large number of workers will generate a large volume of municipal wastes


which may give rise to odor pollution if not properly disposed of.

• SUBCONTRACTOR shall arrange a proper disposal of the wastes.

b) Mitigating Measures

• Noisy activities should be restricted to daytime only especially near


residential areas if the nighttime noise level cannot be kept below 85 d B A.
• To overcome cultural differences that exist among the workers, the provision
of common amenities and other facilities to meet the needs of the workers
is necessary to promote congeniality and integrity.
• Solid wastes generated should be collected in proper bins and disposed of
frequently at designated disposal site.

c) Air quality

Potential Impacts Clearing of vegetation cover and earthwork will increase


the suspended dust and reduction of visibility. The impact to residential area
will not be significant as the nearest is located on the plantation, but reduce
visibility can affect the users. Mitigating Measures Exposure and disturbed
ground should be covered or compacted as possible. If necessary, water
spray should be used during dry period to minimize dust dispersion.

d) Noise Pollution

SUBCONTRACTOR will perform the design and the work related to the
project execution will not break the noise level in excess of the requirement
of the project specification, government and local regulation. The noise
exposure limit will be 85 d b A for site personnel and the neighborhood.
Moreover, SUBCONTRACTOR may utilize noise measurements in the
investigation of complaints involving noise. Work hours will be limited to the
daytime with no work at night to minimize noise complaint.
SUBCONTRACTOR will work at night, if there is any permission from
CONTRACTORS. Equipment with motors will be equipped with noise
suppression devices, for example mufflers to reduce noise.

8. EMERGENCY PREPAREDNESS AND RESPONSE

Site Manager shall be responsible to form emergency evacuation organization. Site


Manager shall make initial contact with related authority.

Injury In all cases of injury, the injured person must report to SUBCONTRACTOR Safety
Department. In cases of a severe injury first call Ambulance and proceed as follows:

– Gassing
Do not attempt a rescue. Inform a CONTRACTORS Operator. never into a gas cloud.

– Burns (oil, acid, caustic, steam, fire)


Immerse the affected part in cold water for least 15 minute.

– Injury (falling, crushing etc)


Do not move the patient other than to remove him from a dangerous area.
Apply artificial respiration if the patient has stopped breathing.
Stop bleeding by applying direct pressure over the wound.
– Electrical Shock
Read the charts electrical posted in all switch rooms.
Isolated electric source. Make sure you are insulated – Pull the casualty from the
equipment. When the casualty is clear, if he has stopped breathing apply artificial
respiration.

– Chemical
Where chemical exposure has occurred other than burns above. The action must be
appropriate to the chemical.

9. HSE MANAGEMENT AUDITING

HSE audit will be undertaken to verify compliance with the HSE plan and HSE project
procedure.
A HSE audit schedule will be prepared and maintain by the HSE Coordinator.
