Ahmed has gotten fever for two days. His temperature increases. His parents finally decide
to bring him to the hospital. After arriving in the hospitals, his mother is in the admission
room to register ahmed. After that ahmed is being examined by a doctor helped by a nurse.

Front officer : welcome to commonwealth hospital. Can I help you, mom?

Mrs. Ali : yes, i’d like to register my son. He has gotten fever for two days. And his
temperature gets higher today.
Front officer : Has he ever been hospitalized or visited a doctor here?
Mrs. Ali : Never
Front Officer : Okay, so we need to fill in the admission form for the patient’s medical
record
Mrs.Ali : Sure
Front Officer : What’s your son’s name?
Mrs.Ali :Ahmed
Front Officer : When was he born?
Mrs.Ali : June 15, 2000.
Front Officer : Where is your address?
Mrs.Ali : Saint Avenue street, number 4
Front Officer : May I have your phone number as a parent?

Front Officer : Okay. So the complaint is fever. Anything else?

Nurse : Hello, I’m Jennifer, your nurse. Your name is ahmed, isn’t it?
Ahmed : Yes, you’re right.
Nurse : Okay,Ahmed, what do you exactly feel?
Ahmed : I have gotten fever for two days
Nurse : Let me check your temperature
Ahmed :Sure
Nurse : It is 39 Celsius degree. Do you feel dizzy or faint?
Ahmed : Yes
Nurse : Did you vomit?
Ahmed : No, I didn’t
Nurse : Let me check your blood pressure
Ahmed : please
Nurse : Well the rate is good
Ahmed : What happens with me then?
Nurse :Well, doctor albert will examine you. You will recognize soon. Please follow
me to visit doctor albert.
Checking Vital Signs

a. Conversation : Checking Vital Signs

Ahmed is being examined by the doctor. The doctor asks the nurse to check ahmed’s vital
signs
Nurse : good morning, doctor. This is ahmed
Doctor : Hello, Ahmed. Have a seat, please.
Ahmed : Thanks
Nurse : I have done with the vital sign check, doc. And here is the result.
Doctor : Thanks. So you have been suffering from fever for two days?
Ahmed : Yes, doc
Doctor : well, your temperature is high enough. That’s not really good actually.
Ahmed : I see
Doctor : You don’t feel any freeze actually.
Nurse : Let me check your temperature
Ahmed : sure
Doctor : It is 38 celcius degree. You don’t feel dizzy or faint. That’s an awkward
symptom actually. Lidia, have you checked his blood pressure, respiration
rate, and pulse
Nurse : I have, doc. The result shows that his BP is 90/80.
Doctor : That’s quite low. Let me check you
Nurse : Please come to the examination room, ahmed.
Ahmed : OK
Nurse : please, lay down here.
Ahmed : Ok
Nurse : You do not need to take off your shoes. That’s Ok
Doctor : I need to check your heart and the pressure by using this stethoscope.
Ahmad : Sure
