Staff Information Sheet
The following method statement has been developed to provide a safe system of work and
must be adhered to at all times, any significant deviation from this system must first be
authorised by your manager or safety representative. Please read the entire sheet before
beginning, if you have any questions please contact your manager or safety
representative.

The main hazards to your safety and health are;


a) Injury from slips trips and falls.
b) Electrocution
c) Falls from height
d) Injury from Manual Handling
e) Noise
f) Injury to contractors and members of the public.
g) Impact with vehicles

Preventative Measures you must take;


a) You must be “competent” to carry out the task.
b) Warning signs must be placed at the entrance to the building and work area
c) Visitors and other members of staff are prohibited from entry unless accompanied
by competent person, all visitors issued with personal protective equipment
d) You must read and be familiar with the COSHH Safety Data Sheets.
e) You must be competent to work at height and roadworks
f) You must keep a fire extinguisher nearby during works
g) You must follow all directions given by the project manager or his agents

Personal Protective Equipment you must wear;

c) Safety boots
d) Safety Glasses
e) Hi-Viz
f) Hard Hat

Environmental Protection Measures you must take;


You must dispose of waste and spoil to the designated area or skip provided for waste.
All work will be undertaken to limit impact to the environment. Any incident will be reported to
the Site manager.
Every effort will be made to keep noise, dust and waste levels to a minimum.

General Precautions
Staff & Training
The project will be carried out by staff from Enzo’s Homes Ltd. All members of staff are
experienced and qualified. A site manager will be appointed who will be responsible for
quality and safety. Apprentices and young workers will be supervised and are not allowed to
carry out tasks for which they have not been trained.

PPE
All site workers will wear Safety boots, and protective clothing at all times, other items of
PPE such as eye protection and gloves are available to be worn as and when necessary.

Preparation & Induction


All members of staff required to work on any contract will be given copies of risk
assessments and method statements, any queries or concerns will be raised with the
contract manager who will ensure it is dealt with. Staff will be inducted onto site by the
contract manager or his deputy, and will follow all site rules and safety procedures.

Welfare
Enzo’s Homes Ltd is responsible for providing adequate washing, toilet, drying and
refreshment facilities for staff and sub-contractors, staff and contractors are responsible for
ensuring that such welfare facilities are maintained in a clean and wholesome manner.

First Aid
It is the responsibility of the company to ensure adequate first aid provision for its staff.
Adequate means provision of a trained first aider, suitable first aid equipment and/or the
provision of an appointed person as the minimum.

A trained first aider will be a suitable person who has attended an HSE approved course of
at least four days' duration and he/she will re-train at least every three years on a course of
not less than two days. From 1st October 2009 first aiders will have to attend a 3 day training
course every 3 years and a refresher course every year in between.

An Appointed Person is a person provided by the employer to take charge of the situation
(e.g. to call an ambulance) if a serious injury/illness occurs in the absence of a first aider.
The Appointed Person can render emergency first aid if trained to do so.

Site Access and Egress


The Client is responsible for providing safe access and egress to the site, Enzo’s Homes Ltd
staff will ensure safe access and egress is maintained for themselves and other contractors
in the area they are working in, good standards of housekeeping will be maintained.

Crane Lifts
Whilst crane is in use on site, all staff and visitors are to use temporary access road.
Diversion signs to be placed at main entrance.
All crane lifts are to be risk assessed by a competent person who will ensure that a lift plan is
in place and briefed to everyone involved in the process.
Only trained, competent and certificated persons will be allowed to carry out the crane lifts.

Material Handling
All materials required for site will be unloaded to a designated unloading and storage area
which will be away from the work area as far as is practicable. This area will be kept tidy to
minimise trip hazards. Materials as and when required will be collected from the storage
area to the work area. All staff will take care when handling materials.

Manual Handling
All Enzo’s Homes Ltd employees and contractors have been instructed on the potential
dangers of manual handling, and have received manual handling training. Staff and
contractors will not lift items of tools or equipment beyond their capabilities. Heavy or
awkward items will be broken down into smaller units or dual lifted where this is not possible.
It is the responsibility of the site foreman to identify and control potentially dangerous manual
handling situations as they occur on site on a day to day basis.

Working from Ladders & Step Ladders


All staff have been made aware of the dangers of working from step ladders and have been
instructed on the safe use of ladders. Staff are required to read and understand HSE leaflet
INDG402 the Safe use of Ladders & Stepladders. Only competent members of staff will be
allowed to work from step ladders and it is the responsibility of the site foreman to ensure
conditions are safe before allowing any ladder work to take place.

Ladders shall only be used for light work of short duration or otherwise as a means of access
to scaffolding; ladders will be tied off at the top to prevent slippage

Working at height
Falls from height remain the single biggest cause of death in the construction industry.

All employees have been made aware of the dangers and consequences of falling from
height, all working at height will be properly planned, organised and supervised; only
competent members of staff will be allowed to work at height and it is the responsibility of the

site foreman to ensure conditions are safe before allowing any work at height to take place.
The foreman will carry out a risk assessment before carrying out work at height and put in
place equipment and measures to prevent falls occurring.

Where he can not eliminate the risk of a fall he will put in place measures and equipment to
minimise the distance and consequences of a fall, should one occur.

All company equipment will be inspected on a regular basis and any damaged equipment
will be withdrawn from service immediately.

Scaffold / Mobile Towers & Access to height


Scaffold will be provided and erected by an approved contractor; the top level of scaffold will
stop approx 2 feet from the working level to allow access to the items to be worked on, the
hand rail will extend a minimum 1 metre above the work level. All work will be carried out
from the scaffold; Enzo’s Homes Ltd will ensure a handover certificate is received from the
scaffolding contractor prior to allowing access to the scaffold. During the works access to
the scaffold will be via ladder which will be tied off at the top, ladders will be removed or
boarded off at the end of each shift to prevent unauthorised access. Damaged or unsuitable
ladders will be removed from site immediately. Scaffolding will be netted at the front to
prevent materials from falling and access onto the platform will be via a gate as per SG:4.00
Should mobile towers be used, they will be erected in accordance with manufacturers
instructions by trained and competent staff.

Contractor/Visitor Safety
Enzo’s Homes Ltd will liaise with other contractor’s staff on a day to day basis and ensure
they are aware of the risks present during the works. Staff and contractors will not leave any
area of work in a dangerous condition or with risks to themselves, other contractors, tenants,
or visitors, all tools and equipment will be cleared to secure storage at the completion of
each shift. Scaffold, ladders and any other access to height will be made inaccessible.

Method Statements

Task Description
This method statement describes the work process for Construction of Road Access and
Widening of highway including:
a) Site establishment
b) Contractor warning and safety
c) Finishing

Prior to starting work


Inform Site Manager and other occupants of work commencing and cordon off work area if
practical to do so.
• Ensure Personal Protective Equipment is worn at all times.
• Place Warning signs at the extremes of the work area prior to the work
commencing.
• Physically check the condition of all equipment before and after use

Step by step guides


• The manager/supervisor is responsible for ensuring that all employees are aware of
all hazards associated with working under or near Overhead power lines.
• The employee is responsible for ensuring that they take all reasonable steps to ensure
their own safety.
• The supervisor shall ensure that only an employee who has the required operative
licence shall operate any equipment/plant.
• The supervisor shall ensure that all records of employee’s certification are kept on file
and regularly reviewed.
• Area to be re “CAT” scanned for location of existing buried services and marked in
position.
• Fencing and working areas to be established and maintained throughout the
construction period and to be erected in accordance with NRSWA and Chapter 8
requirements, (allowing for the excavated material to be stored within fenced off
area if required).
• Fencing to be used will include Heras fencing which will be secured by means of
“double clips” on each panel and red and white barrier boards.
• Customer information signs are to be displayed to include the customer contact
number.
• The road, footpath or verge will be pre-cut to a nominal depth as required and
broken out using a combination of the following equipment, depending on its depth
and structural strength and position of exiting utility apparatus- hydraulic breaker,
heavy breaker, light hand tools.
• Remove the road, footpath or verge surface and repeat cable locating scan.

• “CAT” to be used as excavation progresses to locate any unchartered services.
• Materials arising from the excavation will be cast aside within the Heras Fenced
area or placed onto a dumper to be removed from site.
• Surface reinstatement to be as per relevant type of road reinstatement detailed in
the tables below- from HAUC for footpath, road or verge reinstatement details or
requirements of relevant Highway dept.
• Any unknown services that are found by excavation or detection by CAT and
Genny will be marked by the means of pegs and/or paint and recorded on Record
drawings.
• Reinstatement details and size to be recorded, Photo of cleared works area to be
taken after reinstatement and the notice closed down.

Environmental
Ÿ Localised sump pumping is the only type of groundwater control considered
necessary for this activity
Ÿ Any groundwater pumping will be monitored to ensure silt content is kept to a
minimum
Ÿ Ensure all plant in good working order and silenced to current legislation
Ÿ Drip trays to be used under static plant
Ÿ All materials on site shall be stored in accordance with manufacturers
recommendations
Ÿ Any rubbish accumulated by the works shall be disposed of on a regular basis and
deposited in the correctly named skips at the Main site location Green Meadows.
Ÿ Noise to be controlled by using effective exhaust silencers
Ÿ Fuel Spillage Kits to be available on site.
Ÿ Highways to be monitored and periodically swept when required throughout the
working day.
Ÿ All fuels to be carried in steel explosive proof containers.
Ÿ Any specific environmental issues to be detailed on the site specific risk
assessment.
Ÿ All health, safety and Environmental Works to be carried out in accordance with
EHL Policies.

Emergency Procedures
Ÿ Emergency contacts must be available on site and a site emergency plan provided
to deal with issues such as contact with live services, flooding, trench collapse etc
Ÿ In the case of an emergency the first point of contact will be the Site Manager who
would organise the external emergency services such as police/ambulance/fire brigade

Finishing
All tools and equipment will be cleared to secure storage at the end of each shift
