LETTER OF CONSENT FOR INTERVIEW

Research Title: “A Qualitative Study on the Mobile Gaming Addiction amongst Gr.11 STEM
Students”

Researchers: Elaine Joy Aquino, Vladimir Bermal, John Nicholson Delos Reyes, Gem Carlo
Magalong, Justin Thomas Samson

We, the Grade 11 Research Students at Virgen Milagrosa University Foundation Senior High School
are currently conducting a study entitled “A Qualitative Study on the Mobile Gaming Addiction
amongst Gr.11 STEM Students”. This study is being conducted in partial fulfillment of our
requirements for ATS 005 Practical Research 1. The researchers particularly interested in these
main areas: (1) what are the reason why some STEM students are severely addicted in gaming? (2)
How does gaming addiction become a hindrance to STEM students? (3) What are the effects of
gaming addiction in their: (a) Academic performance (b) Physical Health (c) Social life

This research will add to the body of knowledge about Gaming addiction and its effects. We believe
that you are with us in our enthusiasm to finish this requirement as compliance for our moving-up to
Grade 12 and to develop our well-being. We hope for your positive response on this humble matter.

The interview will take around twenty (20) minutes. Your willingness to participate in the interview is
greatly appreciated. Your participation is voluntary. You do not have to answer any questions you
do not want to answer. If at any time you do not want to continue with the interview, you may
decline. Your time and involvement is profoundly appreciated. To maintain the essence of your words
for the research, the researchers will record the information.

The interview will be audio-recorded and the interviewer will take notes. This is done for data
analysis. The recorded audio will be transcribed by the interviewer and kept confidential. All
individual identification will be removed from the hard copy of the transcript. Participant identity and
confidentiality will be concealed using coding procedures.

Excerpts from the interview may be included in the final dissertation report or other later publications.
However, under no circumstances will your name or identifying characteristics appear in these
writings. If, at a subsequent date, biographical data were relevant to a publication, a separate
release form would be sent to you.
