I. OBJECTIVE

A. Reviewing previous lesson or Review of the past Lesson:

What are the hardware tools used in computer?

Introduce the following learning objectives:

Motivation
The Teacher will show an animated comics strip to the

new lesson learner. The learners are given enough time to read
and understand the story in comic strip. After reading
the story the teacher will ask the moral of the lesson.

REQUISITION FORM -Written order or a formal


demand by the user(s) of a good or service (which is
not made available without a specific request) to the
organization's purchase (or stores) department.

Parts of REQUISITION Form


1. Date refers to the date when the requisition form
was made

person who approves the request of the requisitioner

REQUISITION PROCEDURE

1. Write the name and address of the requisitioner


2. Write the date when the form was fill out

Then the students are asked to fill out the requisition
form using the details of every picture

G. Finding practical applications of The students will use and apply the requisition form
concepts and skills in daily living when requesting for a materials and tools or when
purchasing in a hardware or merchandising store.
H. Making generalizations and Requisition forms is a black and white evidence when
abstraction about the lesson you are requesting/ ordering materials and tools

Activity
Direction: Different pictures of materials and tools
will be shown on the projector. Each picture has their
details to provide an information to fill out the
requisition form using Microsoft excel. The students
I. Evaluating the learning are only given 2 minutes and 30 seconds for each
picture and answer the requisition form. (2 Points for
every material’s correct information and 3 points for
the details of the requisitioner and approving
authority.

Assignment:
