On this day, Thursday, the sixteen of May in the year two thousand and
nineteen (16-05-2019), the Amandment of Agreement of Insurance Program
of Indonesia Migrant Workers, is entered by and between the parties:
I. Mr. ABDULLAH UMAR BASALAMAH, in his position as
Chairperson of the INDONESIA MANPOWER SERVICE
ASSOCIATION (APJATI) in this matter acting for and on behalf of
APJATI domiciled in Jakarta and having addressed at Jalan.
Mampang Prapatan Raya No. 126 South Jakarta, hereinafter
referred to as the “FIRST PARTY”
II. Mr. MARTEN PETRUS LALAMENTIK, Direktor of PT.
ASURANSI SINAR MAS. Domiciled in Jakarta and Having
addressed at Plaza Simas, Jl. Fachrudin No. 18, Jakarta Pusat –
10250, in this matter acting in this position as Chairperson of the
insurance consortium of Indonesian Migrant Workers (PMI),
hereinafter referred to as the “SECOND PARTY”
First Party and Second Party hereinafter jointly referred to as the “Parties”.
RECITALS:
a. WHEREAS, the Parties have been bound to each other to the
Agreement of Insurance Program of Indonesian Migran Workers dated
09-01-2019 Number :02/Kons-MPMI/2019 (hereinaftrer referred to as
the “AGREEMENT”)
b. WHEREAS, the Parties intend to amend certain provision of
AGREEMENT and insert new provisions into the AGREEMENT as set
forth below.
 Based on the forgoing matters, the parties agree to enter into this
AMANDEMENT based on the terms and conditions as set out under the
articles herein below:
I. The parties agree to amend and restate in its entirety article 3
of the agreement regarding premium value \, henceforth read
as follow:

PREMIUM VALUE
Pre Placement : amounting Rp. 75.000,-
II. The parties agree to amend and restate in its entirety Article 5
of the Agreement regarding Premium Value, Henceforth Read
as Follow:
ARTICLE 5
TYPE OF RISK AND BAIL
13. risk of running awaya from apjati shelter : realization of
costs incurred by Indonesian Migrant Workers, a maximum of
IDR 15,000,000,-
III. This amendment shall come into force effect from 09-01-2019
IV. This amendment is not separated from agreement
V. If in the future, errors in the terms and conditions of this
amendment are found, such errors shall be duly rectified by the
parties.
This amendment in made in 2 (two) originals, signed by duty representatives
of the parties pursuant to the prevailing laws, each shall bind and having
equal legal enforcement.
