UNIT OUTAGES DURING THE MONTH OF APRIL’ 2019

Unit –X Outages of KTPS V Stage for the month of April’ 2019 (in Chronological order)
Unit Outage on 07.04.2019: (Boiler tripped on "Reheater Protection Operated"
(mal operation of Vacuum breaker due to spurious signals from IPB-II to PROCONTROL DCS of CCA-02 )

At 16:28 hrs., Turbine tripped on "Condenser Vacuum Low" and immediately Boiler
tripped on " Reheater Protection Operated" as HP bypass couldn't open due to low
condenser vacuum. Cyclic variation in condenser vacuum and flickering of fault LEDs
on turbine BTG panel were perceived. On keen observation, it was concluded that
Condenser vacuum breaker is being operated Open & Close continually which wasn't
realized earlier. Spurious signals generated in IPB-II resulted in faulty operation of
condenser vacuum breaker. Fault has been identified in IPB-II FV60 of Bus traffic
director. Faulty card XV7718 in FV60 was replaced.

UNIT OUTAGES DURING THE MONTH OF APRIL’ 2019
Unit –X Outages of KTPS V Stage for the month of April’ 2019 (in Chronological order)
Unit Outage on 23.04.2019: Boiler tripped on "Both ID Fans OFF" & Turbine tripped on "
Mechanical over speed protection operated". (Budidampadu -I & II feeders tripped on distance relay protection)
The maximum Ex-Bus load at KTPS V Stage is 456 MW. The out going feeders
Budidampadu- I & II, Seetharampatnam- I & II, Tie-Line-I & II & Phase shifting
transformer have a maximum load evacuation capacity of 395 MW, 100 MW, 200 MW & 270
MW respectively. At 15:25 hrs. Budidampadu-II tripped on distance relay protection and
suddenly at 16:06 hrs., Budidampadu-I also tripped on distance relay protection.
Immediately "Generator unbalance relay 46A operated" alarm flashed on BTG panel and
Generator circuit breaker opened , however, the reasons for GCB opening were unclear as no
significant alarms or relay pick ups are observed in Generator transformer protection panels.
Turbine tripped on " Mechanical over speed ( 3250 rpm) operated "and Boiler tripped on "
Both ID Fans OFF" due to snag in extending station supply to unit auxiliaries. Soon after
opening of Generator Circuit Breaker (GCB), Station supply is supposed to extend to Unit
auxiliaries i.e., from SA board to US-10A(T) and SB board to US-10B(B). However, due to
some circuitry complications, Station supply to Unit auxiliaries couldn't extend and Boiler
tripped on " Both ID fans OFF"

Unit Outage on 28.04.2019: (Boiler tube leak at Economiser Zone"
'On confirming tube leak at Economiser Zone, Load gradually reduced duly taking oil
support in CD elevation by tripping AB & CD mills. It is planned to trip the boiler on
" Loss of All Fuel " in order to examine Unit protections during planned shutdowns.
At 30 MW load, Turbine hand tripped at 06:55 hrs. duly ensuing opening of HP bypass
>2% followed by cutting off oil guns in CD elevation and Boiler tripped on "Loss of
all Fuel“
The primary cause of tube leak is at 37 mtr. elevation Economiser lower bank Coil No
1, Tube No.14 tube bend got fish mouth open due to flue gas erosion. After completing
rectification works, Hydrostatic test conducted at 190 Kg/cm2 drum pressure and
found one leak at Economizer Upper bank coil No 157 Tube No 17 got Pin hole in weld
joint.

IT DIVISION REPORT FOR THE MONTH OF APRIL’ 2019
Activities under proposal / progress
 Insulation of surveillance camera at VI stage TG floor, V stage floor was done. 34 nos. PTZ cameras out of
48 nos. and 8 nos. fixed cameras out of 30 nos. has been completed so far and installation of cameras at
outdoor stores, dispensary & stage VI is under progress.
 Biometric attendance system for SPF was installed.
 Customization of ZPERMISSIONS T code in SAP for permissions of employees is under progress.
 Indent preparation is under process for surveillance cameras to replace diverted surveillance cameras.
 Survey has been done for installation of cameras at ash pond.
 Purchase order for replacement of industrial public addressing system units at necessary places of coal
plant, units 9 & 10 is raised and material to be supplied.
 Indent preparation for mini OTDR, splicing kit and power meter is under progress.
