COMPLAINT AFFIDAVIT

I, MARITES DATU LEGURPA, married, of legal age, and a resident of Lot 1, Block 3, Filbert
St., Forest Parkhouse Subdivision south, Brgy. Pampang, Angeles, Pampanga, after being sworn into
in accordance to law, do hereby depose and say THAT:

1. On January 16, 2019 me and my family was at Tagaytay, Cavite having a family trip. Me and
my family were trying to find some exotic plants for sale and at the same time having some
quality time together;

2. Around 8 p.m. we decided to have dinner and while we were searching for a place to eat, I
received a text message from an unknown mobile number around 8:31 p.m.;

3. The said text message came from 09051763874, and when I opened it, I was shocked to see
that it was a death threat message. The message stated “Hi bos oras na para manengel
kame ubosin ko kayo lahat” (Hi boss, it’s time to pay, I will end all of you). This alarmed
me very much since there was not a lot of people who knew my number and that I know for
a fact that I had no known enemies;

4. I also feared for the safety of my family since I still had two (2) minor children as well as for
my husband. Thereafter, me and my family hurriedly went home and secured the safety of our
home. The said threat then stopped after that day, but naturally, I was always looking over my
shoulder whenever I go out and that every night I am haunted by the said threat making it
difficult for me to sleep at night;

5. However, after several weeks, or on February 23, 2019, me and my family were again at
Tagaytay, Cavite to purchase more plants for our garden. Coincidentally, I received another
text message from the said number around 2:50 p.m. stating “Oras mo na dame ng genawa
mo kasalanan amen obosen ko pamelya m” (Time’s up, you’ve done so many wrongs
to us I will end your whole family). This time, I was scared stiff as I knew it was not just a
prank or something, but a serious threat towards the life and safety of my family. A screenshot
of the threats sent to me is herein attached as ANNEX “A” and “A-1”;

6. Thereafter, I called my friend from the National Bureau of Investigation (NBI) and I told him
that I would want to file a complaint against the unknown person sending threats and that I
would want to ask for their assistance in investigating and determining who was this person;

7. I was then invited to NBI CAVIDO to formally file my complaint to which I acceded. I
indicated in their Complaint Sheet the number used to send the threats. Upon returning to
Pampanga, I then sought the help of the Philippine National Police (PNP) Pampanga in order
to lodge a complaint and in order to seek protection while my family is at our residence;

8. After a few days of investigation and reconnaissance, I was informed by the NBI that the
perpetrator hid under the name KRES/KRIS LOGTO and was a resident of 362 Tibagin,
Angeles City, Pampanga;

9. However, the threats did not stop there as the perpetrator continued to torment me even as I
was trying to seem courageous and trying to elicit the true identity of the perpetrator;
10. As his whereabouts is identified, it was at this juncture that I am formally filing this Complaint
against JOHN DOE a.k.a. KRES/KRIS LOGTO for GRAVE THREATS under Article
282 of the Revised Penal Code.

I am heretofore attesting to the veracity of the foregoing and affix my signature below to
attest to the abovestated facts and to show that this Complaint is made without fraud, force,
and/or intimidation and is done by my own free will.

Affiant further says naught.
