INTRODUCTION
1.1 PURPOSE

The purpose of this specification document is to provide a detailed overview of Root Cause
Failure Analysis – Reporting System (RCFA – RS), its function and goals. The requirement
regarding system functional, performance and development will be also described in this
specification document.

1.2 SCOPE

Root Cause Failure Analysis – Reporting System (RCFA – RS) is a system that process for
discovering the main cause of a problem and creating recommendations on how to fix the issues
in future. This system will provide solving problem to ensure that the incident being report
thoroughly according to standard operating procedure. In conclusion, RCFA – RS automate
existing process.

Root cause failure analysis process includes investigators (designated RCFA team of the
project) collecting evidence from incident within project and prepare PowerPoint report to
formulate a solution on how to solve the incident and later present to higher executive for them
to review whether to accept and implement suggested solution. RCFA – RS assist investigators
to create the report. Without implementation of RCFA-RS, they need to manage all documents
by themselves. Managing documents includes sorting files/folders of data collection (evidence)
and charts. RCFA-RS not only manage documents for them, the system also stored all related
incidents in the project, generate PowerPoint report, share between team members, log any
changes of report and also provide single-point entry to access evidence documents, create
charts and open charts.

1.5 OVERVIEW

RCFA – RS computerize RCFA processes. Engineers or staff lodge incident report through
system which notified administrators. The report consists of vital information, for example,
location, date of the incident. Lodging report interface provided by RCFA-RS guide engineer
to provide mentioned vital data. When notification receive, administrator assign RCFA
investigators through RCFA Team page which later notified selected investigation team
members. All evidence and investigation outcome will be uploaded to the system and generate
report when finished. According to the Oxford English Dictionary (OUP 2012) Definition of
evidence is the available body of facts or information indicating whether a belief or proposition
is true or valid. The report later review by high executive to approve working solution.
Approved solution will be implemented and rejected solution will be re-proposed.
