FORMAL OFFER OF EXHIBITS

PETITIONER, by counsel, to this Honourable Court most respectfully
submits their amended formal offer of exhibits, in that:

Exhibit “A” – Entry on Police Blotter written by P03 Gilberto Lacorte Jr.
Purpose – This is offered to establish the fact that the crime was reported
immediately after the incident.

Exhibit “B” – Complaint Affidavit of Mrs. Ryzza Mae C. Dizon

Purpose – This is offered to prove that the private complainant filed a case
against the accused for the crime of Murder.

Exhibit “B-1” – Signature of Mrs. Ryzza Mae C. Dizon in the Complaint

Affidavit
Purpose – This is offered to prove that the private witness-complainant, Ryzza
Mae C. Dizon, duly executed the affidavit (Exhibit “B”).

Exhibit “C” - Death Certificate of Alex P. Dizon

Purpose – This is offered to prove the death and cause of death of the victim,
Mr. Alex P. Dizon.

Exhibit “D” – Autopsy Report on Alex P. Dizon

Purpose - This is offered to establish the fact that Alex P. Dizon, through the
examination conducted by Dr. Josefa D. Baluyot, died from the stab wound on
the vital organs such as the left lung and heart. Moreover, this is offered to
submit to the court information of the cause of death of the accused, to
corroborate the testimony of the witness Ryzza Mae C. Dizon and to correlate
the report with the commission of the crime.

Exhibit “D-1” – Signature of Dr. Josefa D. Baluyot in the Autopsy Report

Purpose – This is offered to prove that the Dr. Josefa D. Baluyot, duly executed
the affidavit (Exhibit- “D”).

Exhibit “E” – Balisong as the object weapon
Purposes – This is offered to establish the fact that the said folding pocket knife
belong to the accused, Cardo A. Dalisay, and was the weapon used
to murder Alex P. Dizon.

Exhibits “A”, “B”, “B-1”, “C”, “D”, “D-1” and “E” are found in the
records of this case.

Upon admission of all the exhibits of the petitioner mentioned herein,

petitioner will rest her case;
