AFFIDAVIT OF LOSS

I, ROMY TECSON MANGUMPIT, Filipino, of legal age, married, with present postal
address at Barangay Poblacion, Aurora, Zamboanga del Sur, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am habitually engaged in the job of making repairs to cell phones and/or
mobile phones of different customers who regularly come to my shop, situated in the arcade of
Aurora Public Market, Aurora, Zamboanga del Sur.

2. That, by reason of the nature of my occupation, as such repairman, I purchased a

single motorcycle described as follows: Single Motor block, white Honda XRM 125 with Plate
No 090106, which I have been using as my service in going to Pagadian City, even as far as
Cagayan de Oro City where I used to buy accessories for cell phones and/or mobile phones for
my regular customers, and then back to Aurora, Zamboanga del Sur, and vice versa;

3. That the above-described single motorcycle which I purchased from Saga Motor
Company has been duly registered in LTO-Molave, Zamboanga del Sur, with my non-
professional driver’s license having been issued to me by the Molave LTO. My said non-
professional driver’s license will expire on August, 2018, while my registration of my said single
motorcycle with Official Receipt (O.R.) will expire earlier on June, 2018, respectively.

4. That, however, the document evidencing my current non-professional driver’s license

was inadvertently misplaced among my personal files I kept in our house in Aurora, Zamboanga
del Sur; nevertheless, my registration of my said single motorcycle, including its Official Receipt
(O.R.) are intact and still in my possession.

3. That despite diligent efforts I exerted to locate the exact place where I last stored it,
nevertheless my current non-professional driver’s license could no longer be traced and any
attempt on my part with the help of the immediate members of my family to locate its
whereabouts, the same also proved futile and to no avail; as a consequence thereof, I treat my
said non-professional driver’s license with expiry date: August, 2018, as having been lost
forever beyond discovery.

4. That I have executed this affidavit of loss to establish the veracity and truthfulness of
the foregoing facts for all legal intents and purposes, and in connection with my application for
renewal of my non-professional driver’s license with the LTO, Molave Branch Office, Molave,
Zamboanga del Sur, on August, 2018, yet.

IN WITNESS WHEREOF, I have hereunto set my hand and affixed my signature this 16th
day of April, 2018, at Pagadian City, Philippines.

There is only one Supreme Court from whose decisions all

other courts are required to take their bearings. While most of
the Court's work is performed by its three divisions, the Court
remains one court-single, unitary, complete and supreme.
Flowing from this is the fact that, while individual justices may
dissent or only partially concur, when the Court states what
the law is, it speaks with only one voice. Any doctrine or
principle of law laid down by the court may be modified or
reversed only by the Court en banc [A.M. No. 03-11-30-SC,
June 9, 2005, 460 SCRA 15-16].
