PREFACE

This Experimental Learning Portfolio is a Narrative Report on the conduct of the

SHS Work Immersion in the partner industries. This report encompasses five chapters with

the different components in it. This book contains the experiences of the immersants which

is reflected in the weekly accomplishment report.

The first part of the portfolio consists of the preliminaries on the cover page and

title page. Likewise, it has table of contents, certificates, approval sheet, preface,

acknowledgment and dedication.

The Chapter I focuses on the Introduction of the portfolio. This deals on the

background of the SHS Work Immersion. Chapter II connects the immersants with the

partner industries. This describes the workplace its history, vision and mission, various

programs and projects, field analysis and organizational structure.

The Chapter III discusses the assessment of work immersion program highlighting

the weekly accomplishment report and documentation, Chapter IV exposes the reflections

of work immersion. Chapter V presents the summary of findings, conclusions and

recommendations.

The last part of the portfolio is the Appendices and attachments. This complies with

the resume, parent’s consent, list of activities, certificate of completion, evaluation sheets,

daily time record and insurance ID and the like.

I would like to extend my gratitude to everyone and praise to God who helped me

bring this study into reality and fulfillment. I would like to express my appreciation to the

I am very grateful of GOD, for being my primary source of my knowledge, skills

and strength that I have used in performing my Work Immersion and accomplishing this

Narrative Report.

A special thanks to Mr. Mark Jhon M. Tamanu Ph. D., our Partnership Focal Person

and for being our Work Immersion Teacher for his guidance and support during our Work

Immersion and until we finished this Narrative Report.

I am also blessed to have my ever supportive parents. Mr. Alfredo Almondia and

Mrs. Dina Almondia, who support me financially and morally by giving all my needs and

comfort with love and care for me to finish not only my Work Immersion and Narrative

Report but also Senior High School.

And of course my relatives, siblings, Kuya Jayson, Kuya JR, Kuya Lawrence, Ate

Diana, Princess for the thoughts they’ve imparted during my Immersion.

Most of all my Kuya Angel Bong who’s now in heaven for always inspire and

motivates me to do this.

CHAPTER I

Introduction

One of the goals of the Basic Education Program is to develop in earners the

competencies, work ethic and values relevant to pursuing further education and joining

nation’s target’s, work immersion, required subject has been incorporated into the

curriculum.

This subject will provide opportunities to become familiar with the workplace for

environment simulation and to apply competencies in areas of specialization or applied

subjects in academic work environments.

Work Immersion is one of the course requirements for graduation. A Senior High

School student has to undergo with Work Immersion in an industry that directly relates to

the student’s postsecondary goal. Through Work Immersion, the students are exposed to

and become familiar with work-related environment related to their field of specialization

to enhance their competence. Specially, the students are able to: (I) gain relevant and

practical industrial skills under the guidance of industry experts and workers; (II)

appreciate the importance and application of the principles and theories taught in school;

(III) enhance their skill technical knowledge and skills; (IV) enrich their skills in

communications and human relations; and (V) develop good work habits, attitudes,

appreciation, and respect for work. These prepare them to meet the needs and challenges

of employment or higher education after graduation. (DepEd Order No. 30 s. 2017

CHAPTER II

History of the Workplace

Vision

The Aparri District Hospital a fifty (50) bed level II hospital, equipped with

adequate medical and support capabilities with integrated and attainable delivery health

service system envisions to competently deliver a comprehensive healthcare, together with

the compassionate, competent and communicating staff utilizing resources and cost

containment of medical care though actually participating in multi-functional collaboration

with in the context of local culture and also though the collaboration of the health agency,

local government and community.

Mission

The Aparri District Hospital provides for the population a quality, affordable and

accessible health care services. It aims to promote health and prevent diseases alongside

with its curative and habilitative care to make people responsible for their own health. It

further aims to upgrade its service through closely networking with other health care

providers and in partnership with the people and community as a whole.
