distribution system does not and will not materially adversely affect the safety,
reliability and efficiency of either Hydro One Networks Inc.’s distribution or
transmission systems and meets the technical requirements specified in Appendix F.2 of
the Distribution System Code, in accordance with section 6.2.25 of the Distribution System
Code.

Furthermore, [Insert name of LDC] verifies and confirms to Hydro One that:

(a) the connection of embedded generation facility [Insert H1 CIA Project Reference] to
Insert name of LDC]’s distribution system has been properly commissioned by the
[Insert name of LDC] in accordance with Appendix F.1 of the Distribution
System Code and the requirements specified within the Connection Impact
Assessment performed by Hydro One (the “CIA”); and

(b) the Existing feeder protections are suitable within the stated terms of the CIA.

employee or agent acting on behalf of the LDC. The equivalency letter needs to be signed
by a Professional Engineer registered in the Province of Ontario or an LDC Officer.

Operating Schedule A/D. Unless otherwise specified for “M” designated feeders,
this will be the Network Operating Division, for “F” designated feeders, this will
be the appropriate Provincial Lines Zone Scheduler. Please check your Schedule
for the appropriate contact numbers and email addresses.

 If telemetry (i.e. monitoring) and/or teleprotection is required between Hydro One

Networks Inc. and the embedded generation facility, the LDC shall complete and
submit a Teleprotection Verification Report (TVR).

 The LDC shall complete and submit a Single Line Diagram (SLD).
