1. TITLE

Method Statement – SURVEY WORKS

This document shall be read in conjunction with the specifications set by the
ABS-CBN Qc Media Arts SHS Building.

2. SCOPE OF WORK

2.1 General Description

To set out the methodology to be used for the Survey Works to be


executed in ABS-CBN Qc Media Arts Building.

This method statement covers the methodology for conducting Field


Survey Works to establish the coordinates and elevation of construction
works.

2.2 Plant and Equipment

 Surveyor shall ensure that specifications of said instruments shall be


in accordance to project requirements.

2.3 Submittals

First Balfour shall provide / submit the following documents:

2.3.1 Land Surveyors Qualification (CVs) and certificates


2.3.2 Calibration certificates of survey equipment to be use in the
project.
2.3.3 Survey works computation for survey control points with
corresponding drawings / key plans.

2.4 Site Planning & Preparation

2.4.1 Site Preparation

The survey control points or other reference points given by


ABS-CBN shall be use for true and proper setting out of works.
The correctness of the position, levels, dimensions and
alignment of all part of work shall be ensured in reference to
the above original points, lines and levels. Necessary
instruments, appliances and labour shall be arranged for the
proper setting out of works. All control points/benchmarks, sight
rails, pegs and other things use in setting out work shall be
carefully protective and preserved.

In case there are no available control points within the vicinity


of the project site, the property corner monuments can be used
to establish horizontal control points.

Site coordination with the other contractors shall be arranged


as required.

2.4.2 Site Management Plan & Access Routes

 Necessary work permits from responsible government


agencies must be obtained prior to commencing work.
 Secure a copy of the necessary permits from the client.
 3rd party certifications attesting the reliability of surveying
equipment must be available prior to the start of work.

2.5 Work Methodology

2.5.1 Control of Survey Instruments

On receipt of the survey instruments, the surveyor shall list out


all the items, recording the serial numbers and any other
identification marks.

Instruments shall be stored in a dry and clean lockable cabinet


preferable inside the project warehouse.

Equipment shall be constantly checked in used by the adoption


of good surveying practices by building self checks.

The calibration status of the instruments shall be continuously


monitored. Copy of the inventory and calibration certificates
shall be sent to TSD-GES.

2.5.2 Calibration and Testing of Survey Instrument
Prior to commencement of survey works, the chief Surveyor
shall ensure that all survey equipment such as GNSS, total
stations and automatic levels deployed in the project are within
the calibration period or duly registered.

Automatic Levels shall undergo peg tests every two weeks to


ensure that readings are within allowable errors. Peg test
records shall be compiled by the chief surveyor.

The Chief surveyor shall ensure that all survey equipment will
be recalibrated on or before the expiration of calibration
certificates.

2.5.3 Verification of Survey Control Points


Together with the client, the survey group shall schedule a joint
inspection to familiarize them with the project site and to check
if the survey control points are still undisturbed. All disturbed
control points/benchmarks shall be identified by the client.
Surveyor (First Balfour or Subcontractor) shall verify the
accuracy of the existing permanent benchmarks, control points
and similar reference survey points by performing a closed
traverse survey using the existing survey control points as
traverse stations before beginning the survey works.

The existing benchmarks or control points shall not be changed


or relocated without prior written approval of the client. The
need to relocate permanent benchmarks or control points shall
be reported to the client before proceeding with the setting out
work.

2.5.4 Coordination Meetings of Survey Teams


The Chief surveyor shall conduct daily coordination meetings
(preferably before or after the daily tool box meeting) to orient
the survey teams on their assignments/work area to eliminate
confusion and double handling of work items when the field
survey has commenced.

2.5.5 Establishment of Horizontal and Vertical Controls

All survey works shall be referred to the existing survey control


points within the project site. In case where additional control
points are needed, traverse and levelling surveys shall be done
and the surveyor shall ensure that errors accumulated within the
surveys are within the allowable tolerances as stated in the
DENR Administrative Order 2007 – 29 or the Revised
Regulations on Land Surveys (Article 7 Section 28.b Project
Primary Control):

Horizontal: Linear Error of Closure 5cm/Km or


Relative Error of Closure 1:20,000
Vertical: Allowable error = 12mm (Distance in Km)
Note: Generally flat surface using Auto Level

For areas where automatic Level is not applicable (e.g. very
steep slope or mountainous area), trigonometric levelling shall
be performed using Total stations. Accuracy for this type of
levelling shall not exceed 3cm.

Survey control monuments shall have a standard dimension of


L20cmxW20cmxH50cm with 30-40cm embedded in the
ground. In case of hard ground, monuments shall be a cement
putty type with standard dimension of L20cmxW20cm.

2.5.6 Topographic Survey

The survey group shall ensure that a topographic survey shall


be done in all critical areas (Foundation, roads, trenches etc.)
before the start of earthwork activity.

All visible features will be measured using ground ground


measurement points for every feature that needs to be shot.
These points will be spread throughout the project area in
sufficient number to fully cover all features and the entire
ground, and to ensure accurate computation of the contour lines
and elevations. Also, the gathered data shall be the basis for the
earthworks (cut and fill) computations.

All man-made structures such as buildings, roads, culverts,


drainages, bridges, walkways, etc., within the project, site will
also be measured in order to get their ground footprint. All other
elements on the ground, such as plantations, trees (with a
minimum diameter), rivers, streams, etc. will also be included
the measurements.

2.5.7 Lay-out Survey


All survey layouts shall be based on the latest AFC drawings.
The Chief Surveyor shall assist in the distribution of latest
Approved for Construction drawings to the survey teams to
ensure correctness of the survey layouts at the project site.

Building Surveys

The grid lines of the building and reference elevations must be


clearly marked on the forms, retaining walls, slabs, footings,
columns and other part of the structure where it is needed.

There should be a reference elevation (1 meter above the finish


floor level) in every column and walls of the building.

Grid line offsets shall be used to prepare and check formwork’s


prior to concrete pouring.

Road Survey

For the clearing and grubbing activity, road centreline layouts


shall be done at 10 m interval (straight) and 5 meter interval
(curves). Said stakes shall serve as guides to determine the
actual number of trees to be cut. Stakes shall be place at Toe
and the embankment limit to determine extent of earthwork area.

Survey marking shall be in the form of wood stakes with colored


flags (or yellow caution tapes) for easy identification.

For the sub base and base course preparation, a minimum of


three stakes per station (Center, Left and Right) shall be place to
define the slope (crown) and extent of course laying. Necessary
number of stakes shall be placed in order to define road curves.

Elevations for the sub base and base course shall be mark by
0.5 to 1 inch diameter wooden stick or 10 mm rebar with masking
tape to define the finish elevations per station.

Cable Trench and Water Drainage Surveys

Cable trench shall be marked on the ground using wood stakes


with colored flags before the start of excavation.

Drainage canals, crossings and catch basins shall also be


marked on the ground while doing the road works. Batter boards
shall also be placed to ensure correctness of alignments and
elevations.

2.5.8 Preparations of Survey Data


Before the start any activity, e.g. laying of the sub base, base
course, concrete pouring etc., the survey team shall prepare the
necessary survey data which will serves as attachment to
request for Inpections/Acceptance (RFIA) to ensure that
preparations done in accordance with the AFC drawings. Minor
deviations of the AFC vs. the as built survey shall be shown for
the inspector to if preparations are within allowable tolerances.
Survey data shall also be prepared as an attachment to Request
for Information (RFI) forms to support or clarify inquiries made to
the client if needed. Key plans or necessary drawings shall also
be attached to highlight specific areas or details that need
clarification/additional information.

All survey data shall be prepared using the standard survey


forms of First Balfour.

2.5.9 As Built Preparation

On completion of the foundation, major site improvements and


other work requiring surveying services, an as-built survey
showing dimensions, locations, angles and elevation of
construction and site work shall be done. All survey data shall be
the basis for the generation of as-built drawings which will be
prepared and submitted to the client.

2.5.10 Organization, Responsibilities and Supervision

The following staff will be directly involved in the execution of


the works. (Please refer to the PMM007NNP – Duties,
Responsibilities and Authorizations for the detailed description
of responsibilities)

2.5.11 Work Force

The following categories of workmen shall be engage at its


designated area. (Please refer to the PMM007NNP – Duties,
Responsibilities and Authorizations for the detailed description
of responsibilities).

3. HEALTH, SAFETY& ENVIRONMENT

3.1 Risk Assessment

Based on this method statement, the activity will be risk assessed and
documented using the Hazards and Risk Analysis (HRA) process. Please
see Annex 6.1. The HRA shall be disseminated to all involved workers via
tool box talk.
On top of that, a Hazard Identification Tool (HIT) Card will be completed
by each foreman prior to start of the activity to identify work site specific
risks and controls with the participation of the workers.

Minimum Personal Protective Equipment (PPE) required as per First
Balfour procedure i.e. safety helmet, safety goggles, steel toed safety
boots and uniform will be issued to all workers involved in the activity
along with other required PPEs such as face shield, gloves and ear
plugs. Other special PPE may be prescribed as per risk assessment.

Work vest with reflectorized strips shall also be issued to worker to


ensure greater visibility.

3.3 Health and Safety General requirements

a) All ESH regulatory, contractual and project requirements will be


adhered to at all times during the performance of the activity.
b) Tag lines, lifting belts/sling, and shackles must be ready on site and in
good safe condition.
c) Watchmen must be equipped with whistles during the activity.
Whistles are used to signal the operators and other workers.
d) No work shall be started or continued during period of high winds,
heavy fog and adverse weather condition or when vision is impaired
by darkness.

3.4 Health and Welfare Arrangement

a) A nurse should be available at the work site to provide health


monitoring and surveillance.
b) Adequate first aid kit will be made available at the work site with
medicines including those for snake bites.
c) Referral to the nearest partner hospital for work related medical
emergencies.

3.5 Fire Precautions

a) Smoking areas will be designated to avoid lighted cigarette butt being


thrown that can start a fire.
b) Portable fire extinguishers will be made available where applicable
i.e. in fuel tank areas, in equipment cabs, etc.
c) When refuelling chain saw, ensure no spillage of fuel on the ground.
d) Fuel should be placed in safe container with safe cover (10-20 liter
plastic Jerry Can).

3.6 ESH Information to Personnel

a) All workers involved in the activity must go through the proper safety
orientation on how to conduct the activity safely prior to start of work.
b) Based on this method statement, a Job Hazard Analysis (JHA) shall
be prepared for the activity and discussed with workers for
understanding and compliance.
c) Tool box talk shall be conducted by the supervisor prior to start of
each work shift with the assistance of the site ESH personnel.

3.7 Emergency Procedures

a) An emergency response team will be organized to combat incipient


fires. Adequate communication system shall be made available at the
work site. Before tree felling, a way of escape shall be determined
and cleared to permit a quick get-away of workers.
b) A ready transportation shall be made available at the work site to
bring injured person to a clinic/hospital promptly.
c) Accounting of personnel will be conducted after each work shift.

4. ENVIRONMENTAL CONCERNS

a) All reasonable steps will be taken to protect the environment. Spillage


of fuel to ground will be avoided.
b) All equipment shall be properly maintained to avoid emissions to
atmosphere.
c) Any chemical spill will be reported immediately.
d) Drip trays will be provided for vehicle / mobile equipment while being
repaired where there is possibility of oil to be released.
e) Discovery of hazardous materials such as unexploded vintage bomb
shall be reported to the client and appropriate governmental agencies
for proper handling.

5. QUALITY ASSURANCE & QUALITY CONTROL

5.1 Contractual Requirements

 Maintain adequate supervision.


 Must comply with the minimum mandatory government
requirements.
 Comply with the client requirements.

5.2 Critical Works

 Ensure that all permits are secured prior to start of activity.
 Ensure that all heavy equipment to be used were certified by a
third party agency.
 Ensure that proper work requests were forwarded to the client for
their information.
 Inspection shall cover the (identify all activities as laid out in the
ITP)

5.4 Follow up and Evaluation

The follow up and evaluation will be made through routine inspection and
preparation of reports.
