1. Proceed to the college where you want to seek admission in, starting June 17, 2019 . Failure to do so will mean
forfeiture of your slot.
2. Present the following documents:
a. Senior High School Card (Form 138)
b. CFAT Rating (this notice)
c. Two ID pictures(1 ½ x 1 ½)
d. Authenticated Birth Certificate from PSA
e. Certification from the Principal that you belong to the upper 40% of the graduating class (for BSN only).
f. Certification of good moral character from the Principal or Guidance Counselor.
g. Scholarship documents, if any.
3. The admitting officer will determine your eligibility based on program-specific admission requirements. If you do not
meet these requirements, you will be advised to seek admission to other programs.
4. After securing your College Admission Slip from the Dean, you may proceed for your medical / physical examination
at the designated venue as indicated below.
5. On the scheduled enrolment for your program, present the following documents to the Registration Officer:
a. Admission Slip from the College where you are admitted
b. Certification from the University Physician indicating physical and mental fitness
