JUDICIAL AFFIDAVIT

(Offer: The testimony of the witness LAURENCE ARADOR BOLINA is being presented in the
Judicial Affidavit as part of his direct testimony, to prove:

1. that he is one of the biological children of the Petitioner and is an adoptee in this instant
case.
2. and other matters relevant and relative to this petition for adoption.

WITNESS is LAURENCE ARADOR BOLINA Filipino, of legal age, single and a resident of Marilima,
Virac, Catanduanes.

1. Q: Mr. Witness, do you know the parties in the case? (Kilala mo ang partido sa kasong ito?)
A: Yes, Ma’am, the Petitioner is my father. And I am his second biological son and one of the
adoptees in this case. (Opo. Ang Petitioner ay aking ama, at ako ang kanyang pangalawang
anak at isa sa mga aampunin sa kasong ito.)

2. Q: Why it is necessary for your father to adopt you and your two siblings? (Bakit kailangan
kayong ampunin ng inyong ama?)
A: In order to legitimize our status. (Para maging legal na anak nya kami.)

3. Q: Why did you and your siblings have an illegitimate status, if you know? (Bakit hindi naging
lihitimo ang katayuan ninyong magkapatid, kung alam mo?)
A: Because when we were born, our mother, unknowingly, was still married to Joey C. Bonagua,
to whom she was married at the age of 15, on the instance of their respective parents, who
alleged in said marriage that she was already 18 years old. (Noong ipinanganak po kaming
magkakapatid, hindi alam ng aming ina na kasal pa rin siya kay Joey C. Bonagua, kung
kanino siya ikinasal noong 15 taong gulang pa lang siya, sa kagustuhan ng kani-kanilang
magulang, at pinalabas na siya ay 18 taong gulang na.)

4. Q: Are your biological parents married? (Kasal ba ang iyong mga natural na magulang?)
 A: Yes, Ma’am. (Opo, Ma’am.)

5. Q: When did they get married? (Kelan sila ikinasal?)

A: They first got married on December 29, 1995 at Sariaya Quezon and got married again on
May 1, 2017, in Virac, Catanduanes. (Una po silang ikinasal noong December 29, 1995 sa
Sariaya Quezon at nagpakasal uli noong May 1, 2017 sa Virac, Catanduanes.)

6. Q: Why do they have to get married again, if you know? (Bakit kailangan nilang magpakasal
uli?)
A: Because of the existence of my mother’s first marriage. So after they found out that the first
man she was married to had died on August 21, 2014, my biological parents legitimated
their invalid relationship on May 1, 2017 in the Parish Church of Magnesia, Virac,
Catanduanrs, before Fr. Joey Aguilar. (Dahil sa unang kasal ng aking ina. Kaya noong
malaman nila na namatay na noong Agosto 21, 2014 ang unang lalaking pinakasalan ng
aking ina, ginawa nilang lehitimo an gang kanilang hindi wastong relasyon noong Mayo 1,
2017, sa simbahan ng Magnesia, Virac, Catanduanes, sa harap ni Fr. Joey Aguilar.)

7. Q: Do you freely and voluntarily give your consent for this adoption? (Malaya ka ba at kusang
loob na ibinigay ang iyong pahintulot para sa pag-aampon na ito?)
A: Yes, Ma’am. (Opo, Ma’am.)

8. Q: What good will this adoption bring you? (Anong kabutihan ang maidudulot ng pag-aampon
na ito sa inyo?)
A: I firmly believe that this adoption will work to our best interest and welfare in order to
legitimize our status. (Naniniwala po ako na ang pag-aampon na ito saamin ay para sa
aming kabutihan at para maging lihitimo na kaming anak.)

9. Q: Does your mother also agree with this adoption? (Pumayag din ba ang iyong ina sa pag-ampon
na ito?)
A: Yes, ma’am. (Opo, Maam.)

10. Q: When this interview was being undertaken, was there anybody present who coached or
influenced you in giving your answer? (Habang ginagawa ang interbyu na ito, marron bang
taong naroon at nag-impluwensya sa iyo sa mga sagot mo?)
A: None, Ma’am. (Wala po, Ma’am.)

11. Q: Was the interview done in English or Tagalog? (Ang pagsisiyasat ba ay ginawa sa salitang
English o Tagalog?)
A: In English translated into Tagalog and Bicol, Maam. (English na Tinagalog at Bicol po,
Ma’am.)

12. Q: Did you willingly, freely and voluntarily participate in this interview? (Kusang loob ka bang
nakilahok sa interbyu na ito?)
A: Yes, Ma’am. (Opo, Maam.)

13. Q: Are you aware that you may be charged for perjury if you are found to have made false
statements in this Judicial Affidavit? (Alam mo ba na maari kang kasuhan ng
pagsisinungaling kapag napatunayan na may ginawa kang kasinungalingan sa salaysay na
ito?)
A: Yes, Ma’am, that was explained by attorney. (Opo, ipinaliwanag po iyan ni Attorney.)
14. Q: Under what circumstances are you executing this Judicial Affidavit? (Sa anong pagkakataon
mo ginawa ang salaysay na ito?)
A: I am executing this Judicial Affidavit under the supervision of Atty. Susan U. Ordinario at her
office in San Jose, Virac, Catanduanes, on August 10, 2019 at around 3:00 o’clock in the
afternoon and she conducted the interview and faithfully recorded the questions asked and
the answers I gave. (Ginawa ito sa pangangasiwa ni Atty. Susan U. Ordinario sa kanyang
opisina sa No. 3 Rizal Ave., San Jose, Virac, Catanduanes, noong Agosto 10, 2019 ng alas-
3 ng hapon at ang lahat ng kanyang katanungan at aking kasagutan ay kanyang isinusulat
at siyang nakalad sa itaas.)
