Level 2B Fieldwork, March 2019 to May 2019
• Managed a full-case load which included the following: complete documentation, leading full
treatment sessions, implementation of assessment tools for re-evaluations, documented with FIM
and CARE tool for reassessment of ADL's, patient and family education, recommendation of medical
equipment, and participation in team meetings with OT and PT.
• Created and presented a visual impairment presentation.
• Gained clinical experience in acute care setting and ICU.
Motus Rehabilitation
Level 2A Fieldwork, January 2019 to March 2019
• Specializing in hand therapy, I gained experience with use of assessment tools, manual therapy,
physical agent modalities, education of proper body mechanics during therapeutic activities,
education on splinting, and utilized functional, meaningful activities to drive occupational therapy
intervention.
