TASK PERFORMANCE

Instructions:

1. This task performance is merged to the Community Outreach Program Implementation of National
Service Training Program (GE1801 - Civic Welfare Training Service 1/GE1802 - Literacy Training
Service 1).
2. Since the planning and revision of the outreach program for the course GE1801 - Civic Welfare
Training Service 1/GE1802 - Literacy Training Service 1 already started in Preliminary Period, your
section should be briefed first by the instructor and/or the students of the said course regarding the
objectives and needs of the program that they designed.
3. You will be graded based on how you help them during the implementation of the outreach program.
Full cooperation and coordination with the instructor and students of CWTS 1/LTS 1 are imperative for
the success of this project.
4. The outreach program that will be drafted and implemented should provide students meaningful
learning experiences that will:
a. enhance and develop a student’s civic consciousness, social ethics, patriotism, and love of country;
b. demonstrate empathy to the less fortunate members of the community; and
c. develop a student’s sensitivity to the feelings and learning needs of others.
5. The portfolio should contain the following:
a. Photocopies of the Participant Evaluation Form accomplished by the attendees/beneficiaries of the
outreach program. (The original copies of the form may be needed by the students of CWTS 1/LTS
1.)
b. Images and photos of the outreach program
c. Reflection paper of every Program Enabler/Student in your section.

6. Portfolio constitutes 30% of your total score, the implementation of the outreach program is 40%, and
the oral presentation during the post-mortem evaluation is 30%.
Sample computation:
o (Score of the portfolio) x (.30) + (Score of the outreach program implementation) x (.40%) +
(Score of the oral presentation) x (.30) = Total Score
o 75 x 0.30 + 80 x 0.40 + 70 x 0.30 = 75.5
