This form must be filled out in its entirety (Pg.1 & 2), signed and included with each firearm you are
shipping along with a copy of your current driver’s license. We will be shipping the firearm received back
to the address of your driver’s license, we cannot ship to a P.O. box. Please contact me directly to get an
Estimated Completion Date by calling the number on this form. We also request that you provide the
tracking number of your shipment via phone.

Payment and Shipping

We accept payments anytime during the project process, but Full Payment will be due upon Project Completion.
We accept Money Orders or Cashiers Checks, Note: Credit/Debt cards will be charged 3% of total order.
Once Payment is received your project will be shipped back to you, there is NO FFL Required but will require an
adult signature to receive it.

Projects not paid in full after 30 days from completion date will be charged 20% Storage fees of Invoiced total for
every 30 days up to 120 days, after 120 days from completion date and through extensive attempts to contact you
via Phone, email and written letter, the project (firearm) will be considered abandoned and The Gun Painter LLC
reserves the right to log the firearm into its company inventory and considered property of The Gun Painter LLC.

If you ship disassembled parts we require you to include a completed inventory list, The Gun Painter LLC will also
validate all parts received and contact you with any discrepancies.

LIABILITY RELEASE AND RISK ASSUMPTION

I agree that I am voluntarily requesting the services provided by THE GUN PAINTER LLC including but not
limited to gunsmithing, coating, modification and or alterations to my/the firearm. I agree that these
alterations may void the factory warranty of the firearm and take full responsibility of this action. I hereby
agree and accept that said activities to be performed on the firearm knowing with full knowledge of the
dangers involved which include but not limited to personal property damage, mechanical failure, bodily
injury and or death and assume any and all risks. In reference to the services being performed, I hereby
agree, for myself, personal representatives, my heirs, and assigns, to release, indemnify, hold harmless and
forever discharge THE GUN PAINTER LLC, and its agents, employees, officers, and principles, of and from
any and all claims, demands, debts, contracts, expenses, lawsuits, causes of action, damages and liabilities,
of every kind of nature, whether known or unknown, in law or equity, that I ever had or may have, arising
from or in any way related to work performed on my/the firearm, even if caused by the negligence of THE
GUN PAINTER LLC, and/or its employees, agents, officers, and/or principles.
