law, do hereby depose and say:

1. That I am employed at Mayorga National High School as a Regular Permanent

Secondary School Teacher since January 5, 2015 to the present;

2. That enclosed is my original certificate of employment issued by my employer;

and

3. That this affidavit is being executed to attest to the foregoing facts and not for the
purpose of FRAUD but in connection with my employment abroad.
