Greetings of peace!

I, Florian D. Villagracia, a Teacher I at Oriental Mindoro National High School (OMNHS) would like
to transfer seven (7) days from my maternity leave which has started last July 22, 2019 to my
husband, Ronell C. Villagracia, Teacher I at Community Vocational High School (CVHS).

The maternity leave is equivalent to 105 working days dating from July 22, 2019 to November 6,
2019. I have already availed the said leave however, due to unexpected caesarean section during
my delivery I would need much help from my husband for I cannot perform certain activities due
to the recent operation I had thus, I would like to transfer seven days to him which will then be
diminished from the said 105 days. Instead of having November 6, 2019 as the last day of my leave
I would be back on October 30 2019.

I am more than willing to report at once to my school on the said date as agreed upon the
conditions of the maternity leave and continue the duties and responsibilities I have left for the
whole duration of my leave.
