Site  Notes:

Any  delays  experienced  by  the  authorised  service  provider  due  to  the  system  not  being  in  the  state  
indicated  by  this  form,  and  therefore  not  being  ready  when  the  service  provider  attends  site  -­  shall  be  
chargeable  and  the  purchase  order  that  has  been  provided  above  will  be  utilised  to  cover  all  out  of  
pocket  expenses  (if  applicable),  till  such  time  as  the  system  is  fully  pre-­  commissioned.    
   
The  authorised  service  provider  is  under  no  obligation  to  remain  on  site  until  pre-­commissioning  work  
is  completed,  but  if  desired  a  purchase  order  may  be  provided  to  the  service  provider  for  such  time  
spent  on  site  due  to  delays  due  to  the  system  not  being  fully  ready  to  be  commissioned.      
   
It  is  the  installing  contractor's  responsibility  to  have  the  necessary  tools,  flow  metering  device  or    calibrated  
gauges  available  -­  for  the  commissioning  agent  to  validate  upon  arriving  on  site.  
-­     Scheduling   of   Commissioning   will   only   occur   once   a   copy   of   this   completed   form   is   received   by   Smardt.

-­  Please  make  allowances  for  a  minimum  of  7  days  from  receipt  of  this  form  by  Smardt  till  the  Date  of  
Commissioning,  to  allow  time  for  the  coordination  of  Smardt's  Service  Partners.

-­  The  Warranty  Period  commences  for  12  months  from  the  date  of  Commissioning,  or  18  months  from  
date  of  shipment,  whichever  occurs  first,  and  is  not  related  to  the  contractors  Defects  Liability  Period.
