Sir,
I have the honour to state the following points for your kind consideration and favourable action
please.

I have come on transfer to Kottayam SSA on 15 Jun 2018 after serving for 2 years at GM
office Thiruvalla in Pathanamthitta SSA. Presently I am posted as JTO (Extnl) at Ettumanoor
Tele. Exchange .

Daily I am traveling 60Kms up and down from my home town Vaikom to reach my exchange.
As a chronic patient of cervical spondylosis on medication my condition is getting aggravated
by daily traveling. Doctor has advised to restrict frequent traveling and avoid strain to the neck
for improving the condition (medical certificate attached). Even I had requested for a home
posting on medical grounds just after completion of one year at GM office Thiruvalla due to the
severity of problem due to long distance traveling. Actually I had requested for a transfer to
my parent SSA basically to get posted to the nearest possible place to my home so as to avoid
daily traveling .

Also I am not in a position to think of staying at Ettumanoor to avoid daily traveling as my

presence at home is very essential especially during night due to the deteriorating health
condition of my ailing and aged parents.

In view of the above facts, I am left with no other option than begging for the mercy of my
administration for helping me out of this crisis. Hence I humbly request your goodness to
consider my case and recommend for a transfer to my home town Vaikom at the earliest, so that
I can take care of my health problems and perform my official duties to the best of my ability.
Also I assure you sir, a home posting can help me in long way to devote more time towards my
official duties and to attend to the emergency needs of my ailing parents.

I shall remain grateful to you ever for your benign disposal.

Sub: Request for clarification on rejection of PF withdrawal request reg:-

Sir,

My requests for fund withdrawal from PF A/c No. KMTD1464 was rejected twice in
the month of Oct 2018. The first request was made on 05/10/2018 for Rs. 30000/- (claim
No.0000962430) and the second was on 20/10/2018 for Rs. 25000/- (claim No.000967557). I
have an amount of Rs. 58384/- as current account balance but still both the applications were
rejected. The amount was urgently required as my house repair work (damaged during the recent
flood) is interrupted due to shortage of fund.

Hence I humbly request you sir please clarify the reasons for rejecting my withdrawal
application and advice me for any correction if needed from my part so that I can get the fund
released as early as possible.
