WAIVER FORM
I voluntarily agree to participate in the 2nd Club Stiga Central Open Developmental Age Group Tournament and knowingly
assume any and all risks of loss, damage to my person or property, injury (including death), both foreseen and unforeseen, of
my attendance at and participation in 2nd Club Stiga Central Open Developmental Age Group Tournament, from any cause
whatsoever, including the fault or negligence of Releasees. I, for myself, my heirs, personal representatives and assigns do
hereby release, waive, discharge and covenant not to sue Club Stiga Central., Topcoms Marketing Corporation, the organizers
and officials, the Facility from all liability, loss, claims, demands, possible causes of action, court costs, settlement costs and
fees, attorneys fees and any other expenses arising from any claim or lawsuit that may arise from any loss, damage or injury
(including death) to me or my property resulting from or arising in connection with, or related to, my attendance at or participation
in the 2nd Club Stiga Central Open Developmental Age Group Tournament. In the event that I am injured, I hereby consent to
the provision of necessary and appropriate emergency medical treatment.
By entering this competition, I grant the 2nd Club Stiga Central Open Developmental Age Group Tournament a limited license to
use my name, likeness, image, voice, video, athletic performance, biographical and other information, in any format whatsoever,
and to distribute, broadcast and exhibit these without charge, restriction or liability, but only for the purposes of advertising or
promoting the sport of Athletics. In no event, however, will such usage constitute an endorsement of any product or service
without my specific written consent.

 TOURNAMENT PROSPECTUS

Title: 2nd Club Stiga Central Developmental Age Group Table Tennis Tournament.

Period: May 19 and 20, 2018.

Venue: Club Stiga Central, TCE Mall Taft Ave. corner Gil Puyat Ave. Pasay City.

Authority: Club Stiga Central Technical Committee.

Rules of Play: This tournament shall be conducted in accordance with 2017 ITTF Laws of Table Tennis and the
Regulation for International Competition herein modified and Supplemented by these guidelines.

Event:

1. Boy’s 10 years old under (Born on or after May 19, 2008)

2. Girl’s 10 years old under (Born on or after May 19, 2008)

3. Boy’s 12 years old under (Born on or after May 19. 2006)

4.Girl’s 12 years old under (Born on or after May 19, 2016)

5. Boy’s 14 years old under (Born on or after May 19, 2004)

6. Girl’s 14 years old under (Born on or after May 19, 2004)

7. Boy’s 16 years old under (Born on or after May 19, 2002)

8.Girl’s 16 years old under (Born on or after May 19, 2002)

9. Boy’s 18 years old under (Born on or after May 19, 2000)

10. Girl’s 18 years old under (Born on or after May 19, 2000)

Eligibility:

1. Member of the of Top four (5) school of UAAP and NCAA and Winner of the Makati Quarterly Age Group
(and other tournament similar in nature) and played for DepEd Regional Meet and Palarong Pambansa, including 1st
and 2nd place of the previous CSC Age Group is INELIGIBLE to compete.

2. Player must submit a photocopy of Birth Certificate issued by PSA on or before May 19, 2018. Note: Player
who submitted their Birth Certificate at last CSC Tournament need not to submit it anymore.
System of Play:

1. All players shall be drawn to each group.

2. Each group shall compete in a single round robin match.

3.Top 2 rank players shall advance to second stage.

4. Second stage, Players shall compete in a Single elimination.

5. All matches shall be three (3) out of five (5) games.

Entries: Player can join a maximum of 2 age group.

Deadline of Entries: May 15, 2018, 8pm.

Awards and Prizes:

1 Champion Medal and Items from STIGA

1 1st Runner up Medal and Items from STIGA

2 2nd Runner up Medal and Items from STIGA

Tournament Fee:

1. Club Stiga Member Fee - 200 pesos

2. Non Member Fee - 300 pesos

Equipment: Colored Blue, Stiga Tables.

Tournament Ball: Colored White, Stiga 3 Star Perform, ITTF approved.

Any unforeseen circumstances, Tournament committee deserve the right to change these guidelines without prior
notice.
