1. For Identification
o Government issued ID card
o Latest ID photo (1″ x 1″ or 2″ x 2″)
o Any one (1) of the following:
o 3 months payslips
o Latest Income Tax Return (ITR)
o Certificate of Employment with details on salary, position and length of
service (original if OFW)
2. Source of Repayment:
o Land Based – Certificate Of Employment with e-mail of employer Sea Based –
Latest POEA Contract
o For Self Employed – 6 months bank statement with BOO Certification of “No
Overdraft Charges”
o Latest Income Tax Return
o 3 months of pay slips (original)
o 6 mos. proof of remittance (bank soa/ passbook)
3. Collateral Requirements
o Land Based – Certificate Of Employment with e-mail of employer Sea Based –
Latest POEA Contract
o Transfer Certificate of Title/Condominium Certificate of Title
o Tax Declaration
o House Plans / Bill of Materials / Building Specifications (for construction loans) OR
Contract-to-Sell (CTS) or Reservation Agreement (RA)

What are the required onboarding documents and where should I submit?

1. Secretary’s Certificate (for corporates) or Partnership Resolution (for partnerships) or DTI Permit (for
sole proprietorship)
2. Signed Electronic Banking Terms & Conditions (EBTC)
3. Completed System Administrator Designation Form

completion, you may submit to your Branch of Account or to your handling Relationship
Manager/Sales Officer.
