GUIDELINES / PROCEDURES ON ACCREDITATION OF OCCUPATIONAL SAFETY

AND HEALTH PRACTITIONER / CONSULTANT


(Pursuant to D.O. 16, SERIES 2001)
QUALIFICATIONS:
A. Practitioner in Occupational Safety and Health
1. Must have completed the 40-hour Basic Occupational Safety and Health Training Course as
prescribed by the Bureau, conducted by BWC accredited or recognized safety training
organizations or institutions;

2. Must be duly licensed to practice his / her profession in the Philippines;

3. Must have relevant experience in occupational safety and health (OSH):


a. at least three (3) years experience if the applicant is a graduate of any engineering or
health related course with PRC license;
b. at least four (4) years experience if the applicant is a graduate of any 4 or 5 year college
course with or without a PRC license;
c. At least ten (10) years experience if the applicant is a college undergraduate.
c.1 An aggregate of 80 hours of Bureau recognized training or other related learning
processes in various fields of OSH may be considered as one (1) year experience, and a
maximum of 160 hours only or equivalent to two years.

4. Must possess the minimum qualifications stated under Rule 1964.01 nos., (1) – (5) of the
OSH Standards, whichever is applicable.

Note: Relevant experience in occupational safety and health means experience in any of the
sub-components in a field of specialization as enumerated in Annex “A of D.O. 16 series of
2001.

B. Consultant in Occupational Safety and Health:


1. Must be an accredited practitioner in occupational safely and heath for at least five (5) years
with experience in at least two (2) fields of specialization enumerated in Annex “A” of D.O.
16 series of 2001;

2. Must have experience in policy formulation and development, monitoring and evaluation,
and implementation of occupational safety and health management systems;

3. Must have completed the 80-hour Advanced Occupational Safety and Training Course as
prescribed by the Bureau; and

4. Must have earned additional 480 hours of training/seminar or related learning processes in
various fields of occupational safety and health from accredited organizations or institutions
authorized by laws.
II. REQUIREMENTS
The following documents must be submitted according to the following order, duly labeled and
placed in a long folder.

1. Application letter as practitioner or consultant in occupational safety and health addressed to


the Bureau Director;

2. Resume (duly signed in all pages) including the following information:


A. Name and Address and contact number of applicant
B. Personal data, specifying the following information: Sex, Civil Status, Type of
Employment, Birth date, Birthplace, Citizenship, Religion, Height, Weight, Blood type,
SSS/GSIS No. and TIN)
C. Educational Background
D. Employment Background (Name of Company, position, status of appointment Inclusive
dates)
E. List of OSH Trainings / Seminars attended (Title, Date, Number of hours, Venue,
Conducted by)
F. Licenses/examinations/eligibilities passed
G. Skills specialization
H. Other qualifications, skills related to OSH

3. Two (2) Copies (one (1 x 1) and one passport size) of most recent I.D. picture with red
background for SP and blue background for SC signed by the applicant at the back;

4. Photocopy of certificate of completion on the BWC prescribed Course on occupational safety


and health;
40-hours Basic Occupational Safety and Health Course for SP applicant
80-hours Advanced Course on OSH for SC applicant.

5. Original copy of certificate of employment indicating present position, date of appointment


and actual job descriptions/duties and responsibilities, duly signed by the personnel
manager/employer or his/her authorized representative;

6. Photocopies of college diploma or Transcript of Records and/or PRC license/ID (if any);

7. Photocopies of certificates of attendance/participation in any OSH related


trainings/seminars/conventions, etc.;

8. Sample copies of safety reports/program prepared/implemented/accomplishments related to


safety and health;

9. Other documents as maybe required.


III. WHERE TO FILE
Applications may be submitted directly to the BWC or through Labor Standards Enforcement
Division of the DOLE Regional Offices. Documents maybe authenticated by an authorized
LSED staff prior to endorsement to BWC for evaluation.

IV. EVALUATION PROCEDURE

1. The evaluator upon receipt of the application shall evaluate the documents for
completeness, veracity and sufficiency with the use of a checklist.

2. The evaluator shall notify the applicant of the results of evaluation within 5 days upon
receipt of the documents by the evaluator using the fastest available means of
communication (e.g., telephone, facsimile, e-mail or letter.), whether documents are
complete or there are still deficiencies/requirements that need to be complied with or
submitted to the Bureau.

3. An applicant maybe required to appear personally for an interview and to present to the
Bureau evaluator, original copies of documents submitted for authentication purposes (if
said documents were not yet certified as true copy by authorized receiving officer of the
Bureau or RO).

4. Evaluation and approval of documents normally takes 15 days upon submission if


requirements are complete and/or applicant has appeared immediately for interview after
proper notification by the evaluator.

V. PAYMENT OF FEE (S) AND ISSUANCE OF CERTIFICATE OF ACCREDITATION AND


IDENTIFICATION CARD

After evaluation and verification by the Bureau, a certificate of accreditation valid for three
(3) years and an identification card shall be issued to any applicant who has satisfactorily
met/ complied all the requirements specified in this rule, upon payment of the prescribed fee
as provided for under rule 1974 of the Standards as amended by D.O. 9 series of 2001.

Payment Procedure

Step 1. The accredited SP/SC shall first secure from OHSD an assessment form indicating
breakdown of fees to be paid for accreditation. An accredited SP shall pay a total of
P450.00 shall be paid by an accredited SP, while P750 shall be paid by an accredited
SC.
Step 2. Proceed to DOLE Accounting Division for issuance of Order of Payment.

Step 3. Proceed to the DOLE Cash Division for payment of required fee. Upon payment, an
Official receipt shall be issued.

Step 4. Present O.R. to OHSD for recording of O.R. number and date of payment.

Step 5. Accomplish Profile Form prior to issuance of certificate of accreditation.

VI. REGISTRATION WITH THE DOLE REGIONAL OFFICE

Upon approval of application and issuance of certificate, the accredited safety practitioner /
consultant shall register with the concerned DOLE Regional Office through LSED by presenting
the certificate of accreditation issued by the BWC.

VII. REQUIREMENTS FOR RENEWAL

Accreditation for practitioners and consultants shall be renewed every three (3) years upon
compliance of the following:
1. Submission of the updated bio-data with recent passport size and 1x1 pictures.
2. Presentation of original and submission of certified true copies of additional and relevant
occupational safety and health training certificates.
3. Submission of annual performance report as certified by the employer/client/s.
4. Payment of renewal/annual fee.
